摘要:
目的 了解三级综合医院护理人员死亡与临终关怀态度现状及其影响因素，为医院提升临终关怀服务水平提供参考和依据。方法 于2020年5月~6月采用便利抽样法，对南充市4家三级综合医院的568名临床护士，采用一般资料调查表、《死亡态度描绘量表》和《佛罗梅尔特临终关怀态度量表》通过问卷星进行调查，分析护理人员临终关怀态度与死亡态度的现状及其影响因素。结果 三级综合医院护理人员死亡态度总分（100.82±18.31）分，5个维度中死亡恐惧得分最高（3.46±0.63）分，其次是自然接受（3.45±0.65）分、趋近接受（3.19±0.70）分、逃避接受（3.16±0.78）分和死亡逃避（3.04±0.79）分；护理人员临终关怀态度总分为（88.76±5.85）分，照顾临终患者积极态度（30.91±3.07）分，以家庭为中心的照顾意识（57.85±10.66）分；单因素分析显示，护理人员临终关怀态度受护理岗位和收入水平的影响（P＜0.05）；相关性分析显示，死亡态度中死亡恐惧、自然接受、趋近接受、逃避接受四个维度与临终关怀态度存在相关性（P＜0.05）；多元线性回归分析显示，死亡恐惧、死亡逃避、自然接受和逃避接受是临终关怀态度的独立影响因素。结论 三级综合医院护理人员对临终关怀态度及服务水平尚有待改善和提高，需进一步加强对护理人员的死亡教育和临终关怀教育，树立正向死亡态度，提升临终照护态度的积极性和临终照护质量。
Abstract:
Objective To understand the current status of death and hospice care attitudes of nursing staff in tertiary general hospitals and their influencing factors, so as to provide reference and basis for the hospital to improve the level of hospice care.Methods From May to June 2020, the convenience sampling method was used to survey 568 clinical nurses in 4 tertiary general hospitals in Nanchong City.Using general information questionnaire,"Death Attitude Delineation Scale"and"Fromelt Hospice Attitude Scale"to investigate the status quo and influencing factors of nursing staff’s hospice attitude and death attitude through questionnaires.Results The total score of the death attitude of the nursing staff in the tertiary general hospital was (100.82±18.31). Among the five dimensions, the score of fear of death was the highest (3.46±0.63).Followed by natural acceptance (3.45±0.65) points, approaching acceptance (3.19±0.70) points, avoidance acceptance (3.16±0.78) points and death avoidance (3.04±0.79) points;The total score of nursing staff’s hospice care attitude was (88.76±5.85) points, the positive attitude of caring for dying patients (30.91±3.07) points, and the family-centered care consciousness (57.85±10.66) points;Univariate analysis showed that nursing staff’s hospice care attitude was affected by nursing position and income level (P<0.05);Correlation analysis showed that the four dimensions of death fear, natural acceptance, approaching acceptance, and evasive acceptance in death attitude were correlated with hospice care attitudes (P<0.05);Multiple linear regression analysis showed that death fear, death avoidance, natural acceptance and avoidance acceptance were independent factors influencing the attitude of hospice care.Conclusion The attitude and service level of nursing staff in tertiary general hospitals need to be improved and improved.It is necessary to further strengthen the death education and hospice care education of nursing staff, establish a positive attitude towards death, and improve the enthusiasm of the end-of-life care attitude and the quality of end-of-life care.
