摘要:
目的 观察左西孟旦与米力农在心脏瓣膜置换术围术期的心肌保护作用。方法 选取2018年10月~2020年10月我院行心脏瓣膜置换术的64例患者为研究对象，采用随机数字表法分为对照组和观察组，各32例。对照组围术期给予米力农，观察组给予左西孟旦，比较两组不同时间段心肌缺血再灌注指标、心功能指标、心脏自动复跳率、术后恢复时间指标。结果 观察组术后12、24、48 h cTnⅠ、LDH、CK-MB均低于对照组，差异有统计学意义（P＜0.05）；观察组LVEF高于对照组，NT-proBNP、LVESD均低于对照组，差异有统计学意义（P＜0.05）；观察组心脏自动复跳率为93.75%，高于对照组的81.25%，差异有统计学意义（P＜0.05）；观察组术后拔管时间、ICU留置时间、住院时间均短于对照组，差异有统计学意义（P＜0.05）。结论 左西孟旦与米力农在心脏瓣膜置换术围术期均有一定的心肌保护作用，但是相比较而言，左西孟旦效果更为有效，心功能改善更佳，心脏自动复跳率高，可缩短康复时间。
Abstract:
Objective To observe the myocardial protective effect of levosimendan and milrinone during the perioperative period of heart valve replacement surgery.Methods 64 patients who underwent heart valve replacement surgery in our hospital from October 2018 to October 2020 were selected as the research objects. They were divided into control group and observation group by random number table method, with 32 cases in each group.The control group was given milrinone during the perioperative period, and the observation group was given levosimendan. The two groups were compared with different time periods of myocardial ischemia reperfusion index, cardiac function index, heart automatic recovery rate, postoperative recovery time index.Results The cTnⅠ, LDH, and CK-MB of the observer at 12, 24, and 48 h after operation were lower than those of the control group,the difference was statistically significant (P<0.05);LVEF of the observation group were higher than those of the control group, and NT-proBNP,LVESD were lower than the control group, the difference was statistically significant (P＜0.05);The automatic recovery rate of the heart in the observation group was 93.75%, which was higher than 81.25% in the control group,the difference was statistically significant(P＜0.05);The postoperative extubation time, ICU retention time, and hospital stay in the observation group were shorter than those in the control group,the difference was statistically significant(P<0.05).Conclusion Levosimendan and milrinone have certain myocardial protection in the perioperative period of heart valve replacement surgery, but in comparison, the effect of levosimendan is more effective, the heart function is improved better, and the heart returns automatically. The high rate can shorten the recovery time.
