摘要:
目的 探讨会阴无保护联合无痛分娩对促进初产妇自然分娩的影响。方法 选取2019年10月~2020年10月在我院分娩的68例初产妇为研究对象，采用随机数字表法分为对照组和观察组，各34例。对照组采用常规会阴无保护分娩，观察组采用会阴无保护联合无痛分娩，比较两组疼痛评分、第二产程时间、新生儿 Apgar 评分、产后出血量、会阴损伤程度、盆腔器官脱垂分度、新生儿窒息以及产妇产后并发症发生率。结果 观察组疼痛评分、第二产程时间、产后出血量均小于对照组（P＜0.05），新生儿 Apgar 评分与对照组比较（P＞0.05）；观察组会阴完整率高于对照组，Ⅰ度、Ⅱ度裂伤率均低于对照组（P＜0.05）；观察组盆腔器官脱垂分级0度比例大于对照组，Ⅰ度、Ⅱ度比例均小于对照组（P＜0.05）；观察组新生儿窒息率、产妇产后并发症发生率均低于对照组（P＜0.05）。结论 会阴无保护联合无痛分娩可促进产程进展，缩短第二产程时间，降低疼痛评分，减少产后出血量，提高会阴完整率，减少盆底功能损伤，降低新生儿窒息和产妇产后并发症发生率，可有效的促进自然分娩。
Abstract:
Objective To explore the effect of unprotected perineum combined with painless delivery on the promotion of natural delivery of primiparas.Methods A total of 68 primiparas who gave birth in our hospital from October 2019 to October 2020 were selected as the research objects.Using random number table method, they were divided into control group and observation group, each with 34 cases.The control group used conventional perineal unprotected delivery, and the observation group used perineal unprotected combined painless delivery.The pain scores, time to second stage of labor, neonatal Apgar score, postpartum hemorrhage, degree of perineal injury, pelvic organ prolapse, neonatal asphyxia, and maternal postpartum complications were compared between the two groups.Results The pain score, time of the second stage of labor, and postpartum hemorrhage in the observation group were lower than those in the control group (P＜0.05);The Apgar score of newborns was compared with that of the control group (P＞0.05);The perineum integrity rate of the observation group was higher than that of the control group, and the rates of degrees I and degrees IIlaceration were lower than those of the control group (P＜0.05);The proportion of pelvic organ prolapse grade 0 in the observation group was greater than that of the control group, and the proportions of degrees I and II were lower than those of the control group (P＜0.05);The rate of neonatal asphyxia and postpartum complications in the observation group were lower than those in the control group (P＜0.05).Conclusion Unprotected perineum combined with painless delivery can promote the progress of labor, shorten the time of second labor, reduce pain score, reduce postpartum hemorrhage, improve perineal intact rate, reduce pelvic floor damage, and reduce the incidence of neonatal asphyxia and postpartum complications.It can effectively promote natural childbirth.
