摘要:
目的 探讨支架联合腹腔镜治疗老年梗阻性结直肠癌的临床疗效，评价其应用安全性。方法 收集我院2015年8月-2020年8月收治的60例老年梗阻性结直肠癌患者作为研究对象，根据治疗方案不同分为对照组（n=32）和观察组（n=28），对照组行腹腔镜治疗，观察组先行支架治疗，而后择期联合腹腔镜治疗；比较两组住院时间、手术时间、术中出血量、术后肛门排气时间、术中清扫淋巴结数目及术后并发症发生情况。结果 观察组手术时间、住院时间、术后肛门排气时间及术中出血量小于对照组，术中清扫淋巴结数目多于对照组，差异有统计学意义（P＜0.05）；观察组术后并发症发生率为17.86%，低于对照组的37.50%，差异有统计学意义（P＜0.05）。结论 给予老年梗阻性结直肠癌患者支架联合腹腔镜治疗能够缩短手术时间，减少术中出血量，加快患者恢复，能够有效解除患者肠道梗阻，安全性良好。
Abstract:
Objective To investigate the clinical efficacy of stent combined with laparoscopy in the treatment of obstructive colorectal cancer in the elderly, and to evaluate the safety of its application.Methods A total of 60 elderly patients with obstructive colorectal cancer admitted to our hospital from August 2015 to August 2020 were collected as the research objects.According to the different treatment options, it is divided into a control group (n=32) and an observation group (n=28). The control group receives laparoscopic treatment, and the observation group receives stent treatment first, and then elective combined laparoscopic treatment;The hospitalization time, operation time, intraoperative blood loss, postoperative anus exhaust time, intraoperative lymph node dissection, and postoperative complications were compared between the two groups.Results The operation time, hospitalization time, postoperative anus exhaust time and intraoperative blood loss in the observation group were less than those in the control group, and the number of lymph nodes dissected during the operation was more than that in the control group, the difference was statistically significant (P<0.05);The postoperative complication rate in the observation group was 17.86%, which was lower than 37.50% in the control group,the difference was statistically significant (P<0.05).Conclusion The stent combined with laparoscopic treatment for elderly patients with obstructive colorectal cancer can shorten the operation time, reduce the amount of intraoperative blood loss, and speed up the recovery of the patient. It can effectively relieve the intestinal obstruction of the patient with good safety.
