摘要:
目的 探讨亚低温血液净化技术在心脏瓣膜病术后休克中的应用效果。方法 选取我院2015年12月-2020年12月收治的60例心脏瓣膜病术后休克患者作为研究对象,随机分为常温组和亚低温组,每组30例。所有患者均进行血液净化治疗,常温组患者在血液净化期间进行血滤机加温处理维持常温,亚低温组对血滤机进行亚低温处理,比较两组患者的恢复情况、死亡率、临床指标、MODS评分、APACHE评分以及并发症情况。结果 亚低温组患者的死亡率、CBP时间、机械通气时间和ICU住院时间低于常温组(P<0.05);治疗后第1天,亚低温组SvO2、CVP、MAP优于常温组(P<0.05),治疗第3天亚低温组CI、SvO2、Lac、PAWP、CVP、MAP优于常温组(P<0.05);治疗后第1天,两组MODS评分比较,差异无统计学意义(P>0.05),亚低温组APACHE评分低于常温组(P<0.05);治疗后第3天,亚低温组MODS评分与APACHE评分低于常温组(P<0.05);常温组并发症发生率为33.33%,高于亚低温组的10.00%(P<0.05)。结论 对心脏瓣膜病术后休克患者在血液净化期间对血滤机进行亚低温处理能够提升患者的恢复情况,减少住院时间和死亡率,促进临床恢复,改善患者循环功能,减少并发症的发生。
Abstract:
Objective To explore the application effect of mild hypothermia blood purification technology in postoperative shock of valvular heart disease.Methods A total of 60 patients with postoperative shock from valvular heart disease who were admitted to our hospital from December 2015 to December 2020 were selected as the research objects, and they were randomly divided into a normothermic group and a mild hypothermia group, with 30 cases in each group.All patients underwent blood purification treatment. Patients in the normal temperature group were treated with the hemofiltration machine to maintain normal temperature during blood purification, and the hypothermia group was treated with sub-hypothermia on the hemofiltration machine.The recovery, mortality, clinical indicators, MODS score, APACHE score and complications of the two groups were compared. Results The mortality, CBP time, mechanical ventilation time and ICU stay in the mild hypothermia group were lower than those in the normal temperature group (P<0.05); On the first day after treatment, SvO2, CVP and MAP in the mild hypothermia group were better than those in the normal temperature group (P<0.05).CI, SvO2, Lac, PAWP, CVP, MAP in the mild hypothermia group were better than the normal temperature group on the 3rd day of treatment (P<0.05);On the first day after treatment, there was no statistically significant difference in the MODS scores between the two groups (P>0.05).The APACHE score of the mild hypothermia group was lower than that of the normal temperature group (P<0.05);On the 3rd day after treatment, the MODS score and APACHE score of the mild hypothermia group were lower than those of the normal temperature group (P<0.05);The complication rate in the normal temperature group was 33.33%, which was higher than 10.00% in the mild hypothermia group (P<0.05).Conclusion The treatment of hypothermia on the hemofiltration machine during blood purification in patients with postoperative shock of valvular disease can improve the patient’s recovery, reduce hospitalization time and mortality, promote clinical recovery, improve the patient’s circulatory function, and reduce the occurrence of complications.
