摘要:
目的 探讨原发性肺淋巴瘤（PPL）的CT特征以及与相关疾病的鉴别要点，以期加深影像医师对该病CT征象的认识，提高诊断水平，减少误诊。方法 回顾性分析 2013年10月-2020年11月我院收治的13例经病理确诊的原发性肺淋巴瘤患者的临床及影像资料，分析其CT征象。结果 13例患者中累及双肺7例、单肺6例，跨叶生长5例；CT表现为结节或肿块4例，肺炎样实变6例，混合表现3例；11例出现空气支气管征，7例可见血管造影征，晕征5例，毛刺征4例，小空泡4例，胸腔积液2例；7例行CT增强扫描，均表现为轻-中度不均匀强化，平均CT值增加15~35 Hu。结论 原发性肺淋巴瘤临床症状缺乏特异性，且临床症状与影像学表现不成正相关，其特殊的CT征象具有诊断提示意义。建议临床怀疑原发性肺淋巴瘤的患者应积极行CT引导下肺穿刺活检明确诊断。
Abstract:
Objective To explore the CT characteristics of primary pulmonary lymphoma (PPL) and the key points for distinguishing it from related diseases, in order to deepen the imaging physicians’ understanding of the CT signs of the disease, improve the diagnosis level, and reduce misdiagnosis.Methods The clinical and imaging data of 13 patients with pathologically confirmed primary pulmonary lymphoma admitted to our hospital from October 2013 to November 2020 were retrospectively analyzed, and their CT signs were analyzed.Results Among the 13 patients, 7 cases involved both lungs, 6 cases of single lung, and 5 cases of cross-leaf growth;CT showed nodules or masses in 4 cases, pneumonia-like consolidation in 6 cases, and mixed manifestations in 3 cases;11 cases had air bronchial signs, 7 cases had angiographic signs, 5 cases had halo signs, 4 cases had burr signs, 4 cases had small vacuoles, and 2 cases had pleural effusion;7 cases of enhanced CT scan showed mild to moderate uneven enhancement, and the average CT value increased by 15-35 Hu.Conclusion The clinical symptoms of primary pulmonary lymphoma lack specificity, and the clinical symptoms are not positively correlated with the imaging findings. Its special CT signs have diagnostic prompting significance.It is recommended that patients with clinical suspicion of primary pulmonary lymphoma should actively undergo CT-guided lung biopsy to confirm the diagnosis.
