摘要:
目的 探讨冠心病患者冠状动脉狭窄病变程度与血清高敏C反应蛋白(hs-CRP)水平、同型半胱氨酸(Hcy)水平及血管内皮功能的相关性。方法 选取2018年10月-2020年10月在我科住院的冠脉造影患者199例,根据冠脉造影结果分为冠脉正常组(n=50)、冠脉狭窄单支病变组(n=97)、冠脉狭窄双支病变组(n=28)和冠脉狭窄三支以上病变组(n=24);另依据Gensini积分中位数分为>6分组(n=56)、≤6分组(n=91)和对照组(即冠脉正常组,n=50)。比较各组Gensini积分、血清hs-CRP和Hcy水平、血管内皮功能测定(FMD)数值,分析血清Hcy、hs-CRP、FMD与Gensini积分的相关性。结果 冠脉狭窄三支以上病变组的Gensini积分、血清Hcy和hs-CRP水平均高于冠脉正常组、冠脉狭窄单支病变组和冠脉狭窄双支病变组,冠脉狭窄三支以上病变组FMD值均低于其余三组,差异有统计学意义(P<0.05);冠脉狭窄双支病变组Gensini积分高于冠脉正常组和冠脉狭窄单支病变组,Hcy水平高于冠脉正常组,FMD值低于冠脉正常组,差异有统计学意义(P<0.05)。Gensini积分>6分组血清Hcy、hs-CRP水平高于≤6分组和对照组,FMD值低于≤6分组和对照组,差异有统计学意义(P<0.05);≤6分组血清Hcy水平高于对照组,FMD值低于对照组,差异有统计学意义(P<0.05)。血清Hcy、hs-CRP与Gensini积分呈正相关(r=0.320,P=0.001;r=0.301,P=0.001),FMD与Gensini积分呈负相关(r=-0.263,P=0.001)。结论 冠心病患者血清hs-CRP、Hcy及血管内皮功能与冠状动脉狭窄病变程度密切相关,可作为评估冠心病患者冠状动脉病变严重程度的重要指标。
Abstract:
Objective To investigate the correlation between the degree of coronary artery stenosis and the levels of serum high-sensitivity C-reactive protein (hs-CRP), homocysteine (Hcy) and vascular endothelial function in patients with coronary heart disease.Methods A total of 199 patients with coronary angiography hospitalized in our department from October 2018 to October 2020 were selected. According to the results of coronary angiography, they were divided into normal coronary artery group (n=50), single vessel coronary stenosis group (n=97), double vessel coronary stenosis group (n=28) and more than three vessel coronary stenosis group (n=24); according to the median Gensini score, they were divided into >6 scores group (n=56), ≤6 score group (n=91) and control group (normal coronary artery group, n=50). The Gensini score, serum hs-CRP and Hcy levels and vascular endothelial function (FMD) were compared, and the correlation between serum Hcy, hs-CRP, FMD and Gensini score was analyzed.Results The Gensini score, serum Hcy and hs-CRP levels in the more than three vessel coronary stenosis group were higher than those in the normal coronary artery group, single vessel coronary stenosis group and double vessel coronary stenosis group, and the FMD value in the more than three vessel coronary stenosis group was lower than those in the other three groups (P<0.05); the Gensini score in the double vessel coronary stenosis group was higher than that in the normal coronary artery group and the single vessel coronary stenosis group, the Hcy level was higher than that in the normal coronary artery group, and the FMD value was lower than that in the normal coronary artery group (P<0.05). The levels of serum Hcy and hs-CRP in Gensini score >6 scores group were higher than those in ≤6 scores group and control group, and the FMD value was lower than those in ≤6 scores group and control group (P<0.05); the serum Hcy level in ≤6 scores group was higher than that in the control group, and the FMD value was lower than that in the control group (P<0.05). Serum Hcy and hs-CRP were positively correlated with Gensini score (r=0.320, P=0.001; r=0.301, P=0.001), FMD was negatively correlated with Gensini score (r=-0.263, P=0.001).Conclusion Serum hs-CRP, Hcy and vascular endothelial function in patients with coronary heart disease are closely related to the degree of coronary artery stenosis, which can be used as an important index to evaluate the severity of coronary artery disease in patients with coronary heart disease.
