摘要:
目的 研究全身麻醉与椎管内麻醉对老年骨科手术患者术后精神状态及认知功能的影响。方法 选取2019年8月-2020年8月我院收治的92例老年骨科手术患者为研究对象，采用随机数字表法分为对照组和观察组，各46例。对照组采用全身麻醉，观察组采用椎管内麻醉，比较两组血压、心率、手术指标、麻醉起效时间、MMSE和MoCA评分、不同时间点认知障碍发生率以及临床不良反应情况。结果 两组收缩压、舒张压、心率比较，差异无统计学意义（P＞0.05）；观察组拔管、苏醒、语言恢复以及麻醉起效时间均短于对照组，差异有统计学意义（P＜0.05）；观察组MMSE评分、MoCA评分高于对照组，差异有统计学意义（P＜0.05）；观察组术后3、6 h认知障碍发生率低于对照组，差异有统计学意义（P＜0.05）；观察组不良反应发生率为6.52%，低于对照组的15.21%，差异有统计学意义（P＜0.05）。结论 与全身麻醉比较，椎管内麻醉起效较快，患者苏醒和语言恢复时间短，对精神状态影响较小，可降低认知功能障碍发生率，且临床不良反应少。
Abstract:
Objective To study the effect of general anesthesia and intraspinal anesthesia on postoperative mental status and cognitive function in elderly patients undergoing orthopaedic surgery.Methods A total of 92 elderly patients with orthopedic surgery admitted to our hospital from August 2019 to August 2020 were selected as the research objects. They were randomly divided into control group and observation group, with 46 cases in each group. The control group was given general anesthesia, and the observation group was given spinal anesthesia. The blood pressure, heart rate, surgical indexes, onset time of anesthesia, MMSE and MoCA scores, incidence of cognitive impairment at different time points and clinical adverse reactions were compared between the two groups.Results There was no significant difference in systolic blood pressure, diastolic blood pressure and heart rate between the two groups (P>0.05). The extubation, recovery, language recovery and anesthesia onset time of the observation group were shorter than those of the control group, and the difference was statistically significant (P<0.05). The MMSE score and MoCA score of the observation group were higher than those of the control group, and the difference was statistically significant (P<0.05). The incidence of cognitive impairment at 3 and 6 h after operation in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). The incidence of adverse reactions in the observation group was 6.52%, which was lower than 15.21% in the control group, and the difference was statistically significant (P<0.05).Conclusion Compared with general anesthesia, intraspinal anesthesia has faster onset, shorter recovery time and language recovery time, less influence on mental state, lower incidence of cognitive dysfunction, and fewer clinical adverse reactions.
