摘要:
目的 探讨胃息肉的临床特征及其幽门螺杆菌感染(Hp)的关系,分析不同治疗方法与胃息肉复发的关系。方法 收集2018年10月-2019年10月我院收治的胃息肉病例520例。依据幽门螺杆菌是否感染及治疗分为Hp根治组110例、非Hp根治组100例、非Hp感染组310例。分析胃息肉患者的临床特征(病理类型、部位、大小、单发及多发)与Hp感染的关系以及不同治疗方法与胃息肉复发之间的关系。结果 Hp感染率为40.38%,抗Hp成功率89.09%;增生性息肉和炎性息肉病例的Hp感染率较高;三组不同年龄、性别、息肉部位、息肉个数及大小比较,差异无统计学意义(P>0.05);三组不同病理类型及治疗方法比较,差异有统计学意义(P<0.05);采用抗Hp、内镜下粘膜切除术、内镜粘膜下剥离术治疗的胃息肉复发率分别为7.27%、24.08%、15.19%。结论 增生性息肉和炎性息肉病例的Hp感染率较高,与Hp感染有一定的关系,抗Hp、EMR和ESD方法治疗胃息肉的复发率降低。
Abstract:
Objective To explore the clinical characteristics of gastric polyps and the relationship between Helicobacter pylori infection (Hp), and analyze the relationship between different treatment methods and recurrence of gastric polyps.Methods A total of 520 cases of gastric polyps admitted to our hospital from October 2018 to October 2019 were collected. According to the infection and treatment of Helicobacter pylori, 110 cases were divided into Hp radical group, 100 cases in non-Hp radical group, and 310 cases in non-Hp infection group. The relationship between clinical features (pathological type, location, size, single and multiple) of patients with gastric polyps and Hp infection, and the relationship between different treatment methods and recurrence of gastric polyps were analyzed.Results Hp infection rate was 40.38%, and the success rate of anti-Hp was 89.09%. Hp infection rate was higher in patients with hyperplastic polyps and inflammatory polyps. There was no significant difference in age, sex, polyp location, number and size of polyps among the three groups (P>0.05); There was significant difference in different pathological types and treatment methods among the three groups (P<0.05).The recurrence rates of gastric polyps treated with anti-Hp, endoscopic mucosal resection and endoscopic submucosal dissection were 7.27%, 24.08% and 15.19%, respectively.Conclusion Hp infection rate is higher in patients with hyperplastic polyps and inflammatory polyps, which has a certain relationship with Hp infection. The recurrence rate of gastric polyps treated with anti Hp, EMR and ESD is reduced.
