摘要:
目的 研究关节镜联合富血小板血浆治疗半月板损伤的临床疗效。方法 选取2019年9月-2021年9月我院骨科收治的66例半月板损伤患者,随机分为对照组与观察组,各33例。对照组行单纯关节镜治疗,观察组采用关节镜联合富血小板血浆治疗,比较两组膝关节Lysholm评分、JOA半月板损伤评分、关节液炎性因子[白细胞介素-1(IL-1)、肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)]、血小板衍生生长因子(PDGF)、血管内皮生长因子(VEGF)、转化生长因子-β(TGF-β)、康复效果。结果 术后3个月,两组Lysholm、JOA评分均高于术前,且观察组高于对照组(P<0.05)。两组IL-1、TNF-α、IL-6水平均低于术前,且观察组低于对照组(P<0.05)。术后3个月,两组PDGF、VEGF、TGF-β均高于术前,且观察组高于对照组(P<0.05)。观察组术后康复优良率高于对照组(P<0.05)。结论 关节镜联合富血小板血浆治疗半月板损伤效果显著,可改善患者膝关节功能,下调体内炎性水平,诱导生长因子释放,促进关节功能康复。
Abstract:
Objective To study the clinical efficacy of arthroscopy combined with platelet-rich plasma in the treatment of meniscus injuries.Methods A total of 66 patients with meniscus injury admitted to Department of Orthopedics of our hospital from September 2019 to September 2021 were selected and randomly divided into control group and observation group, with 33 cases in each group. The control group was treated with arthroscopy alone, and the observation group was treated with arthroscopy combined with platelet-rich plasma. The Lysholm score of knee joint, JOA meniscus injury score, joint fluid inflammatory factors [interleukin-1 (IL-1), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6)], platelet-derived growth factor (PDGF), vascular endothelial growth factor (VEGF), transforming growth factor-β (TGF-β), and rehabilitation effect were compared between the two groups.Results At 3 months after operation, Lysholm and JOA scores of the two groups were higher than those before operation, and those of the observation group were higher than the control group (P<0.05). The levels of IL-1, TNF-α and IL-6 in the two groups were lower than those before operation, and those in the observation group were lower than the control group (P<0.05). At 3 months after operation, the levels of PDGF, VEGF and TGF-β in the two groups were higher than those before operation, and those in the observation group were higher than the control group (P<0.05). The excellent and good rate of postoperative rehabilitation in the observation group was higher than that in the control group (P<0.05).Conclusion Arthroscopy combined with platelet-rich plasma in the treatment of meniscus injury has significant effect, which can improve the knee function of patients, reduce the inflammatory level in vivo, induce the release of growth factors, and promote the rehabilitation of joint function.
