摘要:
目的 研究全髋关节置换术与骨折内固定术对股骨颈骨折愈合及术后功能恢复的影响。方法 选取2020年3月-2021年3月在我院诊治的86例股骨颈骨折患者为研究对象,采用随机数字表法分为对照组和观察组,各43例。对照组采用骨折内固定术治疗,观察组采用全髋关节置换术治疗,比较两组手术指标、Harris髋关节功能恢复优良率、生活质量、并发症发生情况及远期不良事件发生情况。结果 观察组手术时间、术中出血量大于对照组,术后下床活动时间、骨折愈合时间、住院时间均短于对照组,差异有统计学意义(P<0.05);观察组Harris髋关节功能恢复优良率为93.02%,高于对照组的83.72%,差异有统计学意义(P<0.05);两组生活质量评分均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05);随访3个月,观察组不良事件发生率低于对照组,差异有统计学意义(P<0.05)。结论 虽然全髋关节置换术治疗股骨颈骨折的手术时间较长,术中出血量多,但患者术后恢复时间短,可早期下床锻炼,且髋关节恢复优良率高,并发症发生率低,可有效预防远期不良事件,是一种可行的治疗方案。
Abstract:
Objective To study the effect of total hip arthroplasty and fracture internal fixation on femoral neck fracture healing and postoperative functional recovery.Methods A total of 86 patients with femoral neck fracture who were treated in our hospital from March 2020 to March 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 43 cases in each group. The control group was treated with fracture internal fixation, and the observation group was treated with total hip replacement. The operation indexes, Harris hip joint function recovery rate, quality of life, complications and long-term adverse events were compared between the two groups.Results The operation time and intraoperative blood loss in the observation group were higher than those in the control group, and the postoperative ambulation time, fracture healing time and hospitalization time were shorter than those in the control group, the differences was statistically significant (P<0.05). The excellent and good rate of Harris hip joint function recovery in the observation group was 93.02%, which was higher than 83.72% in the control group, and the difference was statistically significant (P<0.05). The quality of life scores of the two groups were higher than those before operation, and the observation group was higher than the control group, the difference was statistically significant (P<0.05). The incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). After 3 months of follow-up, the incidence of adverse events in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).Conclusion Although the operation time of total hip arthroplasty for femoral neck fracture is long and the intraoperative blood loss is large, the postoperative recovery time of patients is short, and they can get out of bed early, and the excellent and good rate of hip joint recovery is high, the incidence of complications is low, which can effectively prevent long-term adverse events. It is a feasible treatment scheme.
