摘要:
目的 研究甲氨蝶呤联合米非司酮治疗不同人绒毛膜促性腺激素（β-hCG）水平宫外孕患者的临床效果。方法 选取2018年10月-2021年3月永新县妇幼保健院妇科收治的60例宫外孕患者，按照β-hCG水平的不同分为A组（β-hCG≤2000 U/L）、B组（2000 U/L＜β-hCG≤5000 U/L）、C组（β-hCG＞5000 U/L），均采用甲氨蝶呤联合米非司酮治疗，比较三组治疗效果、孕酮水平、妊娠包块直径、症状消失时间、血β-hCG恢复时间、妊娠包块消失时间、转手术率、输卵管复通率及不良反应。结果 三组治疗总有效率、孕酮水平、妊娠包块直径、症状消失时间、血β-hCG恢复时间、妊娠包块消失时间、转手术率、输卵管复通率及不良反应总发生率比较，差异有统计学意义（P＜0.05）。结论 甲氨蝶呤联合米非司酮对不同β-hCG水平的宫外孕患者均具有确切疗效，且血β-hCG水平越低，效果越明显，安全性越高。
Abstract:
Objective To study the clinical effect of methotrexate combined with mifepristone in the treatment of ectopic pregnancy with different human chorionic gonadotropin (β-hCG) levels.Methods A total of 60 patients with ectopic pregnancy admitted to Department of Gynecology, Yongxin Maternal and Child Health Hospital from October 2018 to March 2021 were divided into group A (β-hCG≤ 2000 U/L), group B (2000 U/L<β-hCG≤5000 U/L) and group C (β-hCG>5000 U/L) according to the different levels of β-hCG. All groups were treated with methotrexate combined with mifepristone. The therapeutic effect, progesterone level, gestational mass diameter, symptom disappearance time, blood β-hCG recovery time, gestational mass disappearance time, conversion rate, fallopian tube recanalization rate and adverse reactions were compared among the three groups.Results There were significant differences in the total effective rate, progesterone level, pregnancy mass diameter, symptom disappearance time, blood β-hCG recovery time, pregnancy mass disappearance time, operation rate, fallopian tube recanalization rate and total incidence of adverse reactions among the three groups (P<0.05).Conclusion The combination of methotrexate and mifepristone has a definite effect on patients with different β-hCG levels of ectopic pregnancy, and the lower the blood β-hCG level, the more obvious the effect and the higher the safety.
