摘要:
目的 研究肌钙蛋白与心肌酶谱联合检测对冠心病诊断。方法 选取2020年1月-2021年6月我院诊治的54例冠心病为观察组,并选取同期我院的体检健康者50例作为对照组。比较两组肌钙蛋白(cTnⅠ)、心肌酶谱指标[肌酸激酶(CK)、肌酸激酶同功酶(CK-MB)、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(α-HBDH)]各指标水平、不同病情以上各指标水平以及各指标诊断冠心病的敏感度和特异度。结果 观察组cTnⅠ、CK、CK-MB、LDH、α-HBDH均高于对照组(P<0.05);Ⅰ级患者cTnⅠ、CK、CK-MB、LDH、α-HBDH均低于Ⅱ级、Ⅲ级、Ⅳ级(P<0.05),且随分级升高各指标呈上升趋势;cTnⅠ、CK、CK-MB、LDH、α-HBDH各指标单独诊断敏感度、特异度均低于各指标联合检测(P<0.05)。结论 测定肌钙蛋白和心肌酶谱指标对冠心病临床诊断具有重要的价值,且可预测病情严重程度。同时肌钙蛋白与心肌酶谱联合检测诊断冠心病敏感度、特异度均较高,可为临床治疗、预后评估发挥积极的参考作用。
Abstract:
Objective To study the combined detection of troponin and myocardial enzyme spectrum in the diagnosis of coronary heart disease.Methods A total of 54 patients with coronary heart disease diagnosed and treated in our hospital from January 2020 to June 2021 were selected as the observation group, and 50 healthy subjects in our hospital during the same period were selected as the control group. The levels of troponin (cTnⅠ), myocardial enzyme spectrum indexes [creatine kinase (CK), creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), and α-hydroxybutyrate dehydrogenase (α-HBDH)], above indexes with different conditions, and the sensitivity and specificity of each index in the diagnosis of coronary heart disease were compared between the two groups.Results The levels of cTnⅠ, CK, CK-MB, LDH and α-HBDH in the observation group were higher than those in the control group (P<0.05). The levels of cTnⅠ, CK, CK-MB, LDH and α-HBDH in grade Ⅰ patients were lower than those in grade Ⅱ, grade Ⅲ and grade Ⅳ (P<0.05), and the indexes increased with the increase of grade. The diagnostic sensitivity and specificity of cTnⅠ, CK, CK-MB, LDH and α-HBDH were lower than those of the combined detection of each index (P<0.05).Conclusion The determination of troponin and myocardial enzymes has important value in the clinical diagnosis of coronary heart disease, and can predict the severity of the disease. At the same time, the sensitivity and specificity of the combined detection of troponin and myocardial enzyme spectrum in the diagnosis of coronary heart disease are high, which can play a positive reference role for clinical treatment and prognosis evaluation.
