摘要:
目的 研究阿替普酶(rt-PA)早期静脉溶栓治疗急性脑梗死(ACI)对其神经功能的影响。方法 选取2019年4月-2021年5月泗洪县分金亭医院神经内科收治的68例ACI病例进行回顾性分析,依据治疗方案进行分组,将采用常规治疗的患者34例设为对照组,采用rt-PA早期静脉溶栓治疗患者34例则设为观察组。比较两组凝血指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)]、氧化炎症反应[白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、丙二醛(MDA)]、神经功能及不良反应。结果 两组PT、APTT、TT指标较治疗前升高,且观察组高于对照组(P<0.05);而两组IL-1β、TNF-α、MDA水平较治疗前下降,且观察组低于对照组(P<0.05);两组NHISS评分较治疗前下降,且观察组低于对照组(P<0.05);观察组的不良反应发生率(14.71%)与对照组(5.88%)比较,差异无统计学意义(P>0.05)。结论 rt-PA静脉溶栓可有效改善ACI患者凝血功能,同时减轻其氧化炎症反应,促进机体神经功能的恢复,且不增加不良反应风险。
Abstract:
Objective To study the effect of early intravenous thrombolysis with alteplase (rt-PA) on neurological function in patients with acute cerebral infarction (ACI).Methods A total of 68 cases of ACI admitted to the Department of Neurology of Fenjinting Hospital of Sihong County from April 2019 to May 2021 were retrospectively analyzed. According to the treatment plan, 34 patients with conventional treatment were set as the control group, and 34 patients with rt-PA early intravenous thrombolysis were set as the observation group. The coagulation indexes [prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT)], oxidative inflammatory reaction [interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), malondialdehyde (MDA)], neurological function and adverse reactions were compared between the two groups.Results The PT, APTT and TT indexes of the two groups were higher than those before treatment, and the observation group was higher than the control group (P<0.05).The levels of IL-1β, TNF-α and MDA in the two groups were lower than those before treatment, and those in the observation group were lower than the control group (P<0.05).The NHISS scores of the two groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05).There was no significant difference in the incidence of adverse reactions between the observation group (14.71%) and the control group (5.88%) (P>0.05).Conclusion rt-PA intravenous thrombolysis can effectively improve the coagulation function of ACI patients, reduce their oxidative inflammatory response, and promote the recovery of neurological function without increasing the risk of adverse reactions.
