摘要:
目的 观察镍钛机用器械联合超声冲洗对慢性根尖周炎根管内毒素的清除效果。方法 选取2020年4月-2021年4月在我院诊治的76例慢性根尖周炎患者作为研究对象,采用随机数字表法分为对照组和观察组,各38例。对照组采用镍钛机用器械治疗,观察组在对照组基础上联合超声冲洗治疗。比较两组临床疗效、根管内毒素、疼痛发生情况以及牙周健康指标[牙龈指数(GI)、菌斑指数(PLI)、出血指数(BI)、牙周探诊深度(PD)]。结果 观察组治疗总有效率为92.11%,高于对照组的81.58%,差异有统计学意义(P<0.05);两组根管内毒素均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);观察组疼痛度0级、Ⅰ级发生率高于对照组,Ⅱ级、Ⅲ级发生率低于对照组,差异有统计学意义(P<0.05);两组GI、BI、PD均小于治疗前,且观察组GI、BI、PD小于对照组,差异有统计学意义(P<0.05)。结论 镍钛机用器械联合超声冲洗治疗慢性根尖周炎的临床疗效较高,根管内毒素清除效果好,疼痛发生率低,且可改善患者牙周健康指标,值得临床应用。
Abstract:
Objective To observe the effect of nickel-titanium rotary instrumentation combined with ultrasonic irrigation on endotoxin removal in root canal of chronic periapical periodontitis.Methods A total of 76 patients with chronic periapical periodontitis who were treated in our hospital from April to April 2020 were randomly divided into control group and observation group, with 38 cases in each group. The control group was treated with nickel-titanium rotary instrumentation, and the observation group was treated with ultrasonic irrigation on the basis of the control group. The clinical efficacy, root canal endotoxin, pain occurrence and periodontal health indicators of the two groups were compared [gingival index (GI), plaque index (PLI), bleeding index (BI) and periodontal probing depth (PD)].Results The total effective rate of the observation group was 92.11%, which was higher than 81.58% of the control group, and the difference was statistically significant (P<0.05). The endotoxin in the two groups was lower than that before treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). The incidence of pain grade 0 and grade Ⅰ in the observation group was higher than that in the control group, and the incidence of grades Ⅱ and Ⅲ was lower than that in the control group, the difference was statistically significant (P<0.05). The GI, BI and PD of the two groups were less than those before treatment, and the GI, BI and PD of the observation group were less than those of the control group, the difference was statistically significant (P<0.05).Conclusion Nickel-titanium rotary instrumentation combined with ultrasonic irrigation in the treatment of chronic apical periodontitis has high clinical efficacy, good root canal endotoxin clearance effect, low incidence of pain, and can improve periodontal health indicators of patients, which is worthy of clinical application.
