摘要:
目的 研究以时间护理为基础的呼吸训练与排痰管理对肺癌手术患者康复的影响。方法 选取2019年8月-2021年8月于抚州市第一人民医院胸外科行手术治疗的76例肺癌患者,按照随机数字表法分为对照组与观察组,各38例。对照组给予常规护理,观察组在其基础上应用以时间护理为基础的呼吸训练与排痰管理,比较两组肺功能指标[肺活量(FVC)、第1秒用力呼气容积(FEV1)、呼气流量峰值(PEF)]、血气分析指标[动脉氧分压(PaO2)、血氧饱和度(SpO2)]、术后恢复情况及术后并发症情况。结果 两组术后FVC、FEV1、PEF均低于术前,但观察组高于对照组,差异有统计学意义(P<0.05);两组术后1周PaO2、SPO2水平均高于术后24 h,且观察组高于对照组,差异有统计学意义(P<0.05);观察组拔管时间、呼吸机使用时间、下床活动时间、术后住院时间均短于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为7.89%,低于对照组的26.32%,差异有统计学意义(P<0.05)。结论 以时间护理为基础的呼吸训练与排痰管理可促进肺癌患者术后肺功能的恢复,有利于血气指标的改善,可进一步缩短患者的术后恢复时间,降低术后并发症风险。
Abstract:
Objective To study the effect of respiratory training and expectoration management based on time nursing on the rehabilitation of patients with lung cancer surgery.Methods A total of 76 patients with lung cancer who underwent surgical treatment in Department of Thoracic Surgery, Fuzhou First People’s Hospital from August 2019 to August 2021 were selected and randomly divided into control group and observation group, with 38 cases in each group. The control group was given routine nursing, and the observation group was given respiratory training and expectoration management based on time nursing. The pulmonary function indexes [vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF)], blood gas analysis indexes [ arterial partial pressure of oxygen (PaO2), blood oxygen saturation (SpO2)], postoperative recovery and postoperative complications were compared between the two groups.Results The postoperative FVC, FEV1 and PEF of the two groups were lower than those before operation, and those in the observation group was higher than those in the control group, and the difference was statistically significant (P<0.05). PaO2 and SPO2 levels in the two groups at 1 week after operation were higher than those at 24 h after operation, and those in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05). The extubation time, ventilator use time, ambulation time and postoperative hospital stay in the observation group were shorter than those in the control group, and the difference was statistically significant (P<0.05). The incidence of complications in the observation group was 7.89%, which was lower than 26.32% in the control group, and the difference was statistically significant (P<0.05).Conclusion Respiratory training and expectoration management based on time nursing can promote the recovery of pulmonary function in patients with lung cancer after operation, which is conducive to the improvement of blood gas indexes, further shortening the postoperative recovery time and reducing the risk of postoperative complications.
