摘要:
目的 观察右美托咪定对脓毒症休克患者在围术期血流动力学和乳酸清除率的影响。方法 选取2018年8月-2020年8月我院收治的104例脓毒症休克患者作为研究对象,以信封法分为对照组与右美托咪定组,每组52例。对照组行常规麻醉,右美托咪定组在对照组基础上联合右美托咪定治疗,比较两组血流动力学以及乳酸清除情况。结果 两组治疗后SOFA评分、TNF-α水平低于治疗前,且右美托咪定组低于对照组,差异有统计学意义(P<0.05);两组治疗后SpO2、HR低于治疗前,MAP高于治疗前,且右美托咪定组SpO2、HR低于对照组,MAP高于对照组,差异有统计学意义(P<0.05);右美托咪定组治疗后1、3 d乳酸清除率高于对照组,差异有统计学意义(P<0.05)。结论 右美托咪定能清除脓毒症休克患者血清中的乳酸,调节血流动力学,减轻各脏器损伤,改善患者器官消耗氧气的状况及细胞缺氧和组织灌注不良的情况。
Abstract:
Objective To observe the effect of dexmedetomidine on hemodynamics and lactate clearance rate in patients with septic shock during perioperative period.Methods A total of 104 patients with septic shock admitted to our hospital from August 2018 to August 2020 were selected and divided into control group and dexmedetomidine group by envelope method, with 52 cases in each group. The control group was given routine anesthesia, and the dexmedetomidine group was treated with dexmedetomidine on the basis of the control group. The hemodynamics and lactate clearance were compared between the two groups.Results The SOFA score and TNF-α level of the two groups after treatment were lower than those before treatment, and the dexmedetomidine group was lower than the control group, the difference was statistically significant (P<0.05). After treatment, SpO2 and HR in the two groups were lower than those before treatment, MAP was higher than that before treatment, and SpO2 and HR in the dexmedetomidine group were lower than those in the control group, MAP was higher than that in the control group, the difference was statistically significant (P<0.05). The lactate clearance rate in the dexmedetomidine group was higher than that in the control group at 1 and 3 d after treatment, and the difference was statistically significant (P<0.05).Conclusion Dexmedetomidine can remove lactic acid in serum of patients with sepsis, regulate hemodynamics, reduce organ damage, and improve the oxygen consumption of organs, cell hypoxia and poor tissue perfusion.
