摘要:
目的 探究后外侧入路及后侧钢板固定腓骨在旋后外旋型踝关节骨折治疗中的应用价值。方法 选取2019年5月-2021年5月天津市西青医院骨二科收治的128例旋后外旋型踝关节骨折患者,按照随机数字表法分为对照组与观察组,各64例。对照组采用外侧入路外侧腓骨钢板内固定治疗,观察组采用后外侧入路后侧腓骨钢板内固定治疗,比较两组手术时间、切口愈合时间、骨性愈合时间、术后并发症、踝关节功能[踝关节评分(Kofoed)、美国矫形外科足踝协会(AOFAS)踝-后足功能评分]、踝关节功能恢复效果。结果 观察组手术时间、切口愈合时间、骨性愈合时间均短于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05);两组术后3个月Kofoed、AOFAS评分均高于术前,且观察组高于对照组(P<0.05);观察组术后6个月踝关节恢复优良率高于对照组(P<0.05)。结论 后外侧入路及后侧腓骨钢板固定治疗旋后外旋型踝关节骨折,可缩短其手术时间,加快愈合速度,且术后并发症少,有利于患者踝关节功能的快速恢复。
Abstract:
Objective To explore the application value of posterolateral approach and posterior plate fixation of the fibula in the treatment of supination-external rotation ankle fractures.Methods A total of 128 patients with supination-external rotation ankle fractures admitted to the Second Department of Orthopaedics, Tianjin Xiqing Hospital from May 2019 to May 2021 were selected and randomly divided into control group and observation group, with 64 cases in each group. The control group was treated with lateral fibular plate internal fixation through lateral approach, and the observation group was treated with posterior fibular plate internal fixation through posterolateral approach. The operation time, incision healing time, bone healing time, postoperative complications, ankle function [ankle score (Kofoed), ankle-hindfoot function score of American Orthopaedic Foot and Ankle Society (AOFAS)] and ankle function recovery effect were compared between the two groups.Results The operation time, incision healing time and bone healing time in the observation group were shorter than those in the control group (P<0.05). The incidence of complications in the observation group was lower than that in the control group (P<0.05). The Kofoed and AOFAS scores of the two groups at 3 months after operation were higher than those before operation, and those of the observation group were higher than the control group (P<0.05). The excellent and good rate of ankle joint recovery in the observation group 6 months after operation was higher than that in the control group(P<0.05).Conclusion Posterior lateral approach and posterior fibular plate fixation in the treatment of supination-external rotation ankle fractures can shorten the operation time, accelerate the healing rate, and reduce postoperative complications, which is conducive to the rapid recovery of ankle function in patients.
