摘要:
目的 观察主动脉内球囊反搏联合介入治疗急性心肌梗死伴心力衰竭的效果。方法 选取2018年4月-2021年4月在我院诊治的72例急性心肌梗死伴心力衰竭患者为研究对象,采用随机数字表法分为对照组和观察组,各36例。两组均给予介入治疗,观察组在介入治疗基础上联合应用主动脉内球囊反搏,比较两组治疗疗效、心功能指标、心功能分级情况以及不良反应发生情况。结果 观察组治疗总有效率为94.44%,高于对照组的83.33%,差异有统计学意义(P<0.05);两组N末端B型利尿钠肽原(NT-proBNP)均低于治疗前,左室舒张末期内径(LVEDd)及左室射血分数(LVEF)均高于治疗前,且观察组NT-proBNP低于对照组,LVEDd、LVEF均高于对照组,差异有统计学意义(P<0.05);观察组Ⅰ级心功能分级比例大于对照组,Ⅱ级、Ⅲ级、Ⅳ心功能分级比例均小于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为5.56%,与对照组的8.33%比较,差异无统计学意义(P>0.05)。结论 主动脉内球囊反搏联合介入治疗急性心肌梗死伴心力衰竭效果确切,可提高治疗总有效率,改善心功能指标,促进心功能恢复,且不会增加不良反应,进一步提示该联合治疗方法有效、安全,值得临床应用。
Abstract:
Objective To observe the effect of intra-aortic balloon counterpulsation combined with interventional therapy on acute myocardial infarction with heart failure.Methods A total of 72 patients with acute myocardial infarction complicated with heart failure who were treated in our hospital from April 2018 to April 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 36 cases in each group. Both groups were treated with interventional therapy, and the observation group was treated with intra-aortic balloon counterpulsation on the basis of interventional therapy. The therapeutic efficacy, cardiac function index, cardiac function classification and adverse reactions of the two groups were compared.Results The total effective rate of the observation group was 94.44%, which was higher than 83.33% of the control group, and the difference was statistically significant (P<0.05). The N-terminal B-type natriuretic peptide (NT-proBNP) in the two groups was lower than that in the treatment, and the left ventricular end-diastolic diameter (LVEDd) and left ventricular ejection fraction (LVEF) were higher than those before treatment, and the NT-proBNP in the observation group was lower than that in the control group, and LVEDd and LVEF were higher than those in the control group, with statistically significant difference (P<0.05). The proportion of cardiac function grade Ⅰ in the observation group was higher than that in the control group, and the proportion of cardiac function grades Ⅱ, Ⅲ and Ⅳ in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05). The incidence of adverse reactions in the observation group was 5.56%, which was compared with 8.33% in the control group, the difference was not statistically significant(P>0.05).Conclusion Intra-aortic balloon counterpulsation combined with interventional therapy is effective in the treatment of acute myocardial infarction complicated with heart failure, which can improve the total effective rate, improve cardiac function indexes, promote the recovery of cardiac function, and do not increase adverse reactions. It further suggests that the combined treatment is effective and safe, and is worthy of clinical application.
