摘要:
目的 探讨老年二型糖尿病患者血清谷氨酰转移酶(GGT)、视黄醇结合蛋白4(RBP4)水平与代谢综合征发生的相关性。方法 选取2019年1月-2021年3月我院收治的70例老年二型糖尿病合并代谢综合征(MS)患者作为研究组,选取同时期来我院体检的健康者70例作为对照组。比较两组一般资料及血清GGT、RBP4水平,分析二型糖尿病合并MS患者血清GGT、RBP4水平与MS各指标的相关性,利用Logistic回归方法分析老年二型糖尿病合并MS的危险因素,评估血清GGT和RBP4水平在诊断二型糖尿病合并MS的灵敏度和特异度。结果 研究组的腰围、收缩压、舒张压、BMI、TG、FPG、LDL-C、HbA1c、GGT、RBP4水平均高于对照组,差异有统计学意义(P<0.05);研究组患者血清GGT及RBP4水平与腰围、收缩压、舒张压、BMI、TG、FPG、LDL-C、HbA1c、GGT、RBP4水平呈正相关,与年龄及HDL-C呈负相关(P<0.05);年龄、腰围,以及BMI、TG、FPG、LDL-C、HbA1c、HDL-C、GGT、RBP4水平均为老年2型糖尿病合并MS的危险因素;血清GGT、RBP4水平联合诊断老年2型糖尿病合并代谢综合征的灵敏度(77.83%)、特异度(70.38%)以及约登指数(0.881)均高于单一指标。结论 血清GGT、RBP4水平与老年2型糖尿病合并代谢综合征密切相关,也是老年2型糖尿病合并MS的危险因素,二者联合检测对于老年2型糖尿病合并MS具有一定的临床诊断价值。
Abstract:
Objective To investigate the relationship between serum glutamyltransferase (GGT), retinol binding protein 4 (RBP4) levels and metabolic syndrome in elderly patients with type 2 diabetes mellitus.Methods A total of 70 elderly patients with type 2 diabetes mellitus complicated with metabolic syndrome (MS) admitted to our hospital from January 2019 to March 2021 were selected as the study group, and 70 healthy people who came to our hospital for physical examination at the same time were selected as the control group. The general data and serum GGT, RBP4 levels were compared between the two groups. The correlation between serum GGT, RBP4 levels and MS indexes in patients with type 2 diabetes mellitus complicated with MS was analyzed. Logistic regression method was used to analyze the risk factors of type 2 diabetes mellitus complicated with MS in the elderly. The sensitivity and specificity of serum GGT and RBP4 levels in the diagnosis of type 2 diabetes mellitus complicated with MS were evaluated.Results The waist circumference, systolic blood pressure, diastolic blood pressure, BMI, TG, FPG, LDL-C, HbA1c, GGT and RBP4 levels in the study group were higher than those in the control group, and the differences were statistically significant (P<0.05). The serum GGT and RBP4 levels in the study group were positively correlated with waist circumference, systolic blood pressure, diastolic blood pressure, BMI, TG, FPG, LDL-C, HbA1c, GGT and RBP4 levels, and negatively correlated with age and HDL-C (P<0.05). Age, waist circumference, and BMI, TG, FPG, LDL-C, HbA1c, HDL-C, GGT, RBP4 levels were risk factors for MS in elderly patients with type 2 diabetes mellitus; the sensitivity (77.83%), specificity (70.38%) and Youden index (0.881) of combined serum GGT and RBP4 levels in the diagnosis of elderly type 2 diabetes mellitus complicated with metabolic syndrome were higher than those of single index.Conclusion Serum GGT and RBP4 levels are closely related to metabolic syndrome in elderly patients with type 2 diabetes mellitus, and they are also risk factors for MS in elderly patients with type 2 diabetes mellitus. The combined detection of GGT and RBP4 has certain clinical diagnostic value for MS in elderly patients with type 2 diabetes mellitus.
