摘要:
目的 探讨糖尿病患者血清前白蛋白(PA)与冠状动脉狭窄程度之间的关系。方法 收集2017年8月-2020年12月因胸痛就诊于深圳市龙岗中心医院并行冠状动脉造影的患者819例,根据冠脉造影结果将患者进行CAD-RADS分级,根据是否患有糖尿病分为糖尿病组(210例)和非糖尿病组(609例),收集两组一般资料、血清PA水平和血糖水平并进行单因素分析,分析两组间的冠状动脉狭窄程度与血清PA水平及血糖之间的相关性。结果 糖尿病组和非糖尿病组性别、年龄、血糖、BUN、TG、HDL-C、LDL-C、及PA比较,差异有统计学意义(P<0.05);两组总蛋白、白蛋白、球蛋白、RBP、UA和Cr比较,差异均无统计学意义(P>0.05);不同CAD-RAS分级患者性别、年龄、总蛋白、白蛋白、球蛋白、PA、RBP、TC、LDL-C、GLU、BUN、Cr比较,差异有统计学意义(P<0.05);不同CAD-RAS分级患者UA、TG比较,差异无统计学意义(P>0.05);糖尿病组血清PA水平与冠状动脉狭窄程度呈负相关(r=-0.155,P<0.05),非糖尿病组血清PA水平与冠状动脉狭窄程度无相关性(r=-0.070,P>0.05);有序Logistic回归分析显示,血清PA(OR=-0.004,95%CI:-0.006~-0.002)、血糖(OR=0.042,95%CI:0.011~0.073)均为冠状动脉狭窄的独立危险因素。结论 糖尿病患者血清PA水平与冠状动脉狭窄程度呈负相关,检测血清PA浓度对预测糖尿病患者冠状动脉病变及其病情进展有一定临床意义。
Abstract:
Objective To investigate the relationship between serum prealbumin and coronary artery stenosis in diabetic mellitus patients.Methods A total of 819 patients who underwent coronary angiography due to chest pain in Shenzhen Longgang Central Hospital from August 2017 to December 2020 were collected. According to the results of coronary angiography, the patients were graded by CAD-RADS. In addition, according to whether they had diabetes, they were divided into diabetic mellitus group (210 cases) and non-diabetic mellitus group (609 cases). The general data, serum prealbumin level and blood glucose level of the two groups were measured and univariate analysis was performed to analyze the correlation between the degree of coronary artery stenosis and serum prealbumin level and blood glucose between the two groups.Results There were significant differences in gender, age, blood glucose, BUN, TG, HDL-C, LDL-C and PA between diabetic mellitus group and non-diabetic mellitus group (P< 0.05); there was no significant difference in total protein, albumin, globulin, RBP, UA and Cr between the two groups (P>0.05). There were significant differences in gender, age, total protein, albumin, globulin, PA, RBP, TC, LDL-C, GLU, BUN and Cr among patients with different CAD-RAS grades (P<0.05); there was no significant difference in UA and TG among patients with different CAD-RAS grades (P>0.05). The level of serum prealbumin in diabetic group was negatively correlated with the degree of coronary artery stenosis (r=-0.155, P<0.05); there was no correlation between serum PA level and coronary artery stenosis in non-diabetic group (r=-0.070, P>0.05). Ordinal logistic regression analysis showed that serum prealbumin (OR=-0.004,95%CI:-0.006- -0.002) and blood glucose (OR=0.042,95%CI:0.011-0.073) were independent risk factors for coronary artery stenosis.Conclusion The level of serum PA in diabetic mellitus patients is negatively correlated with the degree of coronary artery stenosis. The detection of serum PA concentration has certain clinical significance in predicting coronary artery disease and its progression in diabetic mellitus patients.
