摘要:
目的 探讨优质护理与共情沟通干预联合用于妊娠期高血压（妊高症）伴急性肾衰竭（ARF）患者的临床效果。方法 选取2019年6月-2020年7月在本院诊治的56例妊高征并发ARF患者作为研究对象，通过随机数字表法分为常规组和干预组，各28例。常规组行常规护理+优质护理，干预组行常规护理+优质护理+共情沟通干预。比较两组干预4周后临床体征（血压、24 h尿蛋白值）、负面情绪[抑郁自评量表（SDS）、焦虑自评量表（SAS）]、心理弹性（心理弹性测评表）及应对方式[Jabwiece 应对量表（JCS）]变化情况。结果 干预组舒张压、收缩压及24 h尿蛋白定量低于常规组（P＜0.05）；干预组SDS及SAS评分均低于常规组（P＜0.05）；干预组情绪控制力、抗压能力、个人强度评分及沟通能力、愉悦度评分和总评分均高于常规组（P＜0.05）；干预组积极面对、寻求支持及情感宣泄评估得分均高于常规组，自我依赖、屈服及逃避评分低于常规组（P＜0.05）。结论 在妊高症伴ARF患者临床治疗过程中实施优质护理联合共情沟通干预，能够有效提高患者临床体征的改善效果，缓解患者负面情绪，增强其心理弹性，能够有效帮助患者在疾病治疗期间改变自身的应对方式。
Abstract:
Objective To explore the clinical effect of high quality nursing combined with empathy communication intervention on gestational hypertension (PIH) patients with acute renal failure (ARF).Methods A total of 56 patients with pregnancy-induced hypertension complicated with ARF who were treated in our hospital from June 2019 to July 2020 were selected as the research objects. They were divided into routine group and intervention group by random number table method, with 28 cases in each group. The routine group received routine nursing+quality nursing, and the intervention group received routine nursing + quality nursing+empathy communication intervention. The changes of clinical signs (blood pressure, 24 h urine protein), negative emotions [self-rating depression scale (SDS), self-rating anxiety scale (SAS)], resilience (resilience scale) and coping style [Jabwiece coping scale (JCS)] were compared between the two groups after 4 weeks of intervention.Results The diastolic blood pressure, systolic blood pressure and 24 h urine protein quantification in the intervention group were lower than those in the conventional group (P<0.05). The scores of SDS and SAS in the intervention group were lower than those in the conventional group (P<0.05). The scores of emotional control, stress resistance, personal strength, communication ability, pleasure and total score in the intervention group were higher than those in the routine group (P<0.05). The scores of positive face, seeking support and emotional catharsis in the intervention group were higher than those in the routine group, and the scores of self-dependence, surrender and escape were lower than those in the routine group (P<0.05).Conclusion The implementation of high-quality nursing combined with empathy communication intervention in the clinical treatment of patients with gestational hypertension and ARF can effectively improve the improvement effect of patients’ clinical signs, alleviate patients’ negative emotions, enhance their psychological resilience, and effectively help patients change their coping styles during disease treatment.
