摘要:
目的 分析计算机手术辅助系统(CAS)联合电视辅助胸外科技术(VATS)在精准肺段切除术中作用。方法 收集2020年6月-2021年6月在山东第一医科大学第三附属医院胸外科行肺段切除患者16例,先用CAS对肺部病灶三维重建及定位,然后在VATS下行精准肺段切除术,观察患者手术时间、术中出血量、术后胸腔引流量、住院时间等指标数据。结果 在全腔镜下顺利完成16例肺段切除,无围术期死亡,无支气管胸膜瘘,无肺不张等并发症,均顺利出院。平均手术时间(182.21±28.33)min,平均出血量(92.62±36.54)ml,术后胸腔平均引流量(329.63±161.32)ml,平均住院时间(6.22土1.46)d。术后病理诊断良性病变2例;恶性病变14例,原位癌4例,微浸润腺癌6例,浸润性腺癌4例。结论 应用CAS联合VATS技术行解剖性肺段切除,可缩短手术时间,提高手术的安全,促进肺段手术精细化发展。
Abstract:
Objective To analyze the role of computer-assisted surgery (CAS) combined with video-assisted thoracic surgery (VATS) in precise segmentectomy.Methods From June 2020 to June 2021, 16 patients underwent segmentectomy in the Department of Thoracic Surgery, the Third Affiliated Hospital of Shandong First Medical University. CAS was used to reconstruct and locate the three-dimensional lung lesions, and then the operation time, intraoperative blood loss, postoperative thoracic drainage and hospitalization time of precise segmentectomy under VATS were collected.Results Totally 16 cases of pulmonary segmentectomy were successfully completed under total endoscopy. There was no perioperative death, no bronchopleural fistula, no atelectasis and other complications. The average operation time was (182.21±28.33) min, the average blood loss was (92.62±36.54) ml, the average postoperative chest drainage was (329.63±161.32) ml, and the average hospitalization time was (6.22±1.46) d. Postoperative pathological diagnosis of benign lesions in 2 cases; there were 14 cases of malignant lesions, 4 cases of carcinoma in situ, 6 cases of microinvasive adenocarcinoma and 4 cases of invasive adenocarcinoma.Conclusion The application of CAS combined with VATS in anatomical segmentectomy can shorten the operation time, improve the safety of the operation, and promote the fine development of lung segment surgery.
