摘要:
目的 研究丹参川芎嗪联合尼麦角林对后循环缺血性眩晕（PCIV）患者的效果及对基底动脉血流速度的影响。方法 选取2020年3月-2021年9月佳木斯市中心医院收治的40例PCIV患者，采用随机数字表法分为对照组和观察组，各20例。对照组给予尼麦角林治疗，观察组在其基础上应用丹参川芎嗪治疗，比较两组眩晕评定量表（DHI）评分、血液流变学参数（血浆黏度、全血高切黏度、全血低切黏度）、双侧椎动脉（VA）与基底动脉（BA）血流流速、眩晕改善时间、住院时间及复发率。结果 两组治疗后躯体（DHI-P）、情感（DHI-E）、功能（DHI-F）评分低于治疗前，且观察组低于对照组（P＜0.05）；两组治疗后血浆黏度、全血高切黏度、全血低切黏度低于治疗前，且观察组低于对照组（P＜0.05）；两组治疗后双侧VA及BA血流流速高于治疗前，且观察组高于对照组（P＜0.05）；观察组眩晕改善时间、住院时间短于对照组，且复发率低于对照组（P＜0.05）。结论 丹参川芎嗪联合尼麦角林可改善PCIV患者的临床症状，降低其血黏度，增加患者椎-基底动脉血流速度，缩短康复时间，降低复发几率。
Abstract:
Objective To study the effect of Danshen ligustrazine combined with nicergoline on patients with posterior circulation ischemic vertigo (PCIV) and the effect of basilar artery blood flow velocity.Methods Forty patients with PCIV admitted to Jiamusi Central Hospital from March 2020 to September 2021 were selected and divided into control group and observation group by random number table method, with 20 cases in each group. The control group was treated with nicergoline, and the observation group was treated with danshen ligustrazine on the basis of the control group. The vertigo rating scale (DHI) score, hemorheological parameters (plasma viscosity, whole blood high shear viscosity, whole blood low shear viscosity), blood flow velocity of bilateral vertebral artery (VA) and basilar artery (BA), vertigo improvement time, hospitalization time and recurrence rate were compared between the two groups.Results The scores of body (DHI-P), emotion (DHI-E) and function (DHI-F) in the two groups after treatment were lower than those before treatment, and the scores in the observation group were lower than those in the control group (P<0.05). The plasma viscosity, whole blood high shear viscosity and whole blood low shear viscosity of the two groups after treatment were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). After treatment, the blood flow velocity of bilateral VA and BA in the two groups was higher than that before treatment, and the observation group were higher than those in the control group (P<0.05). The improvement time of vertigo and hospitalization time in the observation group were shorter than those in the control group, and those in the recurrence rate were lower than those in the control group (P<0.05).Conclusion Danshen ligustrazine combined with nicergoline can improve the clinical symptoms of PCIV patients, reduce their blood viscosity, increase the blood flow velocity of vertebrobasilar artery, shorten the recovery time and reduce the risk of recurrence.
