摘要:
目的 研究噻托溴铵与异丙托溴铵治疗慢阻肺的效果及对血清炎症因子的影响。方法 选取2020年1月-2021年1月在我院诊治的62例慢阻肺患者为研究对象,采用随机数字表法分为对照组和观察组,各31例。对照组采用异丙托溴铵治疗,观察组采用噻托溴铵治疗,比较两组临床疗效、慢阻肺自我评估测试(CAT)评分、血清炎症因子水平、肺功能指标及不良反应发生率。结果 观察组治疗总有效率为93.55%,高于对照组的80.65%(P<0.05);两组CAT评分均低于治疗前,且观察组低于对照组(P<0.05);两组降钙素原(PCT)、C反应蛋白(CRP)、白细胞(WBC)水平低于治疗前,且观察组低于对照组(P<0.05);两组用力肺活量(FVC)、第1秒用力呼气容积占预计值百分比(FEV1%)大于治疗前,且观察组大于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 噻托溴铵与异丙托溴铵治疗慢阻肺均具有一定的效果,但噻托溴铵的疗效更好,可改善患者肺功能,降低血清炎症因子水平、CAT评分,值得临床应用。
Abstract:
Objective To study the effect of tiotropium bromide and ipratropium bromide in the treatment of chronic obstructive pulmonary disease and its effect on serum inflammatory factors.Methods A total of 62 patients with chronic obstructive pulmonary disease diagnosed and treated in our hospital from January 2020 to January 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 31 cases in each group. The control group was treated with ipratropium bromide, and the observation group was treated with tiotropium bromide. The clinical efficacy, COPD self-assessment test (CAT) score, serum inflammatory factor level, lung function index and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.55%, which was higher than 80.65% of the control group (P<0.05). The CAT scores of the two groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05). The levels of procalcitonin (PCT), C-reactive protein (CRP) and white blood cell (WBC) in the two groups were lower than those before treatment, and those in the observation group were lower than the control group (P<0.05). The forced vital capacity (FVC) and the percentage of forced expiratory volume in the first second (FEV1%) in the two groups were higher than those before treatment, and the observation group was higher than the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Tiotropium bromide and ipratropium bromide in the treatment of chronic obstructive pulmonary disease have a certain effect, but tiotropium bromide is better, which can improve lung function, reduce serum levels of inflammatory factors, CAT score, and is worthy of clinical application.
