摘要:
目的 探讨成人癫痫患者（PWE）的睡眠障碍与情绪调节能力和生活质量的关系。方法 采用横断面研究方法，对2020年11月-2021年11月于安徽医科大学第一附属医院神经内科就诊的120例成人癫痫患者进行匹兹堡睡眠质量量表（PSQI）、中文版癫痫患者生活质量评定量表（QOLIE-31）、中文翻译的情绪调节困难量表（DERS）测评，另招募性别、年龄相匹配的51名健康人群作为对照组完成PSQI和DERS评估，比较成人癫痫患者和对照组的睡眠障碍发生率，探讨成人癫痫患者的情绪调节能力、生活质量、其他人口学和临床特征对睡眠障碍的影响，采用逐步多元线性回归分析成人癫痫患者睡眠障碍的危险因素。结果 120例成人癫痫患者中约51.67%合并睡眠障碍，较对照组更高（20.34%），且患者的情绪调节能力较对照组更差，差异有统计学意义（P＜0.05）；单因素分析显示，成人癫痫患者的婚姻类型、发作类型、发作频率、受教育年限、情绪调节能力及生活质量均与睡眠障碍相关（P＜0.05）；多元逐步回归分析显示，癫痫患者的睡眠障碍与情绪调节困难、生活质量、发作类型和发作频率相关（P＜0.05）。结论 睡眠障碍在癫痫患者中非常普遍，情绪失调、较差的生活质量、全面性起源的发作类型与频繁的癫痫发作共同影响患者的睡眠质量。
Abstract:
Objective To investigate the relationship between sleep disorders and emotional regulation and quality of life in adult patients with epilepsy (PWE).Methods A cross-sectional study was conducted to evaluate the Pittsburgh Sleep Quality Scale (PSQI), the Chinese version of the Quality of Life in Epilepsy Scale (QOLIE-31), and the Chinese version of the Difficulties in Emotion Regulation Scale (DERS) in 120 adult patients with epilepsy who were admitted to the Department of Neurology, the First Affiliated Hospital of Anhui Medical University from November 2020 to November 2021. In addition, 51 healthy people with matching gender and age were recruited as the control group to complete PSQI and DERS assessments. The incidence of sleep disorders in PWE patients and the control group was compared. The effects of emotional regulation ability, quality of life, other demographic and clinical characteristics on sleep disorders in PWE patients were investigated. Stepwise multiple linear regression was used to analyze the risk factors of sleep disorders in PWE patients.Results About 51.67% of 120 PWE patients had sleep disorders, which was higher than that of the control group (20.34%), and the emotional regulation ability of the patients was worse than that of the control group, the difference was statistically significant (P<0.05). Univariate analysis showed that marital type, seizure type, seizure frequency, years of education, emotional regulation ability and quality of life of PWE patients were related to sleep disorders (P<0.05). Multiple stepwise regression analysis showed that sleep disorders in patients with epilepsy were related to emotional regulation difficulties, quality of life, seizure type and seizure frequency (P<0.05).Conclusion Sleep disorders are very common in patients with epilepsy. Emotional disorders, poor quality of life, generalized seizure types and frequent seizures affect the sleep quality of patients.
