摘要:
目的 观察间苯三酚联合缩宫素对自然分娩初产妇促宫颈成熟的效果。方法 选取2020年2月-2021年2月在我院自然分娩的76例初产妇为研究对象，采用随机数字表法分为对照组和观察组，各38例。对照组采用缩宫素进行催产，观察组在对照组基础上联合间苯三酚催产。比较两组促宫颈成熟效果、不同产程时间、疼痛评分、新生儿Apgar 评分、宫颈成熟评分（Bishop）、产后出血量、宫口全开时间以及并发症发生率。结果 观察组促宫颈成熟总有效率为92.11%，高于对照组的81.58%（P＜0.05）；观察组第一产程、第二产程时间均短于对照组（P＜0.05），第三产程时间与对照组比较，差异无统计学意义（P＞0.05）；观察组自然分娩期间疼痛评分低于对照组（P＜0.05），新生儿Apgar 评分与对照组比较，差异无统计学意义（P＞0.05）；两组用药后宫颈成熟评分高于用药前，且观察组高于对照组（P＜0.05）；观察组宫口全开时间小于对照组（P＜0.05），产后出血量与对照组比较，差异无统计学意义（P＞0.05）；观察组并发症发生率为7.89%，低于对照组的18.42%（P＜0.05）。结论 间苯三酚联合缩宫素对自然分娩初产妇促宫颈成熟效果确切，具有较高的有效率，且可减轻产妇疼痛、缩短第一和第二产程时间，缩短宫口全开时间，提高宫颈成熟评分，降低并发症发生率。同时不会影响新生儿Apgar 评分、产后出血量，是一种有效、可行的促宫颈成熟方法。
Abstract:
Objective To observe the effect of phloroglucinol combined with oxytocin on cervical ripening in natural childbirth primipara.Methods A total of 76 primiparas who gave birth naturally in our hospital from February 2020 to February 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 38 cases in each group. The control group was treated with oxytocin, and the observation group was treated with phloroglucinol on the basis of the control group. The effect of promoting cervical ripening, different labor time, pain score, neonatal Apgar score, cervical ripening score (Bishop), postpartum hemorrhage, uterine opening time and complication rate were compared between the two groups.Results The total effective rate of cervical ripening in the observation group was 92.11%, which was higher than 81.58% in the control group (P<0.05). The first and second stage of labor in the observation group were shorter than those in the control group (P<0.05), and there was no significant difference in the third stage of labor between the two groups (P>0.05). The pain score during natural childbirth in the observation group was lower than that in the control group (P<0.05). The cervical ripening scores of the two groups after treatment were higher than those before treatment, and the observation group was higher than the control group (P<0.05). The total opening time of uterine orifice in the observation group was less than that in the control group (P<0.05), and there was no significant difference in postpartum hemorrhage between the two groups (P>0.05). The incidence of complications in the observation group was 7.89%, which was lower than 18.42% in the control group (P<0.05).Conclusion Phloroglucinol combined with oxytocin has a definite effect on promoting cervical ripening in primiparas with natural childbirth, and has a high efficiency. It can reduce maternal pain, shorten the time of the first and second stages of labor, shorten the time of full opening of the cervix, improve the cervical ripening score, and reduce the incidence of complications. At the same time, it will not affect the neonatal Apgar score and postpartum hemorrhage. It is an effective and feasible method to promote cervical ripening.
