摘要:
目的 观察重型颅脑损伤后吞咽障碍患者音乐穴位电刺激联合吞咽功能训练的效果。方法 选取2020年1月-2022年2月在我院诊治的62例重型颅脑损伤后吞咽障碍患者为研究对象,采用随机数字表法分为对照组和观察组,各31例。对照组采用吞咽功能训练,观察组在对照组基础上联合穴位电刺激干预,比较两组吞咽功能恢复效果、洼田饮水试验评分、反复唾液吞咽测试(RSST)评分、摄食-吞咽功能障碍评分、吞咽功能分级情况及生活质量。结果 观察组吞咽功能恢复总有效率为93.55%,高于对照组的80.65%(P<0.05);干预后,两组洼田饮水试验评分、RSST评分、摄食-吞咽功能障碍评分均低于干预前,且观察组低于对照组(P<0.05);干预后,观察组吞咽功能1级、2级占比均高于对照组,3级、4级、5级占比小于对照组(P<0.05);干预后,两组生活质量评分均高于干预前,且观察组高于对照组(P<0.05)。结论 穴位电刺激联合吞咽功能训练可促进重型颅脑损伤后吞咽障碍患者吞咽功能恢复,降低吞咽障碍功能分级和洼田饮水试验、RSST以及摄食-吞咽功能障碍评分,促进吞咽功能恢复,提高患者生活质量水平,具有重要的临床应用价值。
Abstract:
Objective To study the effect of acupoint electrical stimulation combined with swallowing function training on swallowing function in patients with dysphagia after severe craniocerebral injury.Methods A total of 62 patients with dysphagia after severe craniocerebral injury who were treated in our hospital from January 2020 to February 2022 were selected as the research objects. They were divided into control group and observation group by random number table method, with 31 patients in each group. The control group was treated with swallowing function training, and the observation group was treated with acupoint electrical stimulation on the basis of the control group. The swallowing function recovery effect, Watian drinking water test score, repeated saliva swallowing test (RSST) score, feeding-swallowing dysfunction score, swallowing function classification and quality of life were compared between the two groups.Results The total effective rate of swallowing function recovery in the observation group was 93.55%, which was higher than 80.65% in the control group (P<0.05). After intervention, the scores of water swallow test, RSST and feeding-swallowing dysfunction in the two groups were lower than those before intervention, and those in the observation group were lower than those in the control group (P<0.05). The proportion of grade 1 and grade 2 of swallowing function in the observation group was higher than that in the control group, and the proportion of grade 3, grade 4 and grade 5 was lower than that in the control group (P<0.05). The quality of life score of the two groups after intervention were higher than those before intervention, and that in the observation group was higher than that in the control group (P<0.05).Conclusion Acupoint electrical stimulation combined with swallowing function training can promote the recovery of swallowing function in patients with dysphagia after severe craniocerebral injury, reduce the classification of swallowing dysfunction and the scores of water swallow test, RSST and feeding-swallowing dysfunction, improve the effective rate of swallowing function recovery and improve the quality of life of patients, which is worthy of clinical application.
