摘要:
目的 观察改良头皮半冠状切口在颧骨颧弓骨折患者切开复位内固定术中的应用效果。方法 选取2017年7月-2022年7月我院诊治的40例颧骨颧弓骨折患者为研究对象,采用随机数字表法分为对照组和治疗组,各20例。两组均采用内固定钛板进行内固定,对照组选择常规头皮半冠状切口,治疗组采用改良头皮半冠状切口,比较两组临床手术指标(切口长度、术中出血量、手术时间、住院时间)、骨折愈合优良率、双侧颧弓突度差、软组织畸形、张口度、并发症发生率、咀嚼功能评分以及治疗满意度。结果 治疗组手术切口、术中出血量、手术时间、住院时间均小于对照组(P<0. 05);治疗组颧骨颧弓骨折愈合优良率为95.00%,高于对照组的80.00%(P<0.05);治疗组双侧颧弓突度差、软组织畸形均小于对照组,张口度大于对照组(P<0.05);治疗组并发症发生率为5.00%,低于对照组的20.00%(P<0.05);治疗组咀嚼功能评分低于对照组(P<0.05);治疗组治疗满意度为100.00%,高于对照组的85.00%(P<0.05)。结论 在颧骨颧弓骨折切开复位内固定术中选择改良头皮半冠状切口,可减小术中出血量,缩短手术和住院时间,提高骨折愈合优良率,减小双侧颧弓突度差和软组织畸形,增加张口度,降低并发症发生率,促进咀嚼功能恢复,提升患者治疗满意度。
Abstract:
Objective To observe the application effect of modified scalp semi-coronal incision in open reduction and internal fixation for patients with fracture of zygoma and zygomatic arch.Methods A total of 40 patients with fracture of zygoma and zygomatic arch in our hospital from July 2017 to July 2022 were randomly divided into control and treatment groups, with 20 patients in each group. Both groups were treated with internal fixation with titanium plate, while the control group was treated with conventional scalp semi-coronal incision, and the treatment group was treated with modified scalp semi-coronal incision. The clinical operation indexes (incision length, intraoperative blood loss, operation time, hospitalization time), excellent and good rate of fracture healing, poor bilateral zygomatic arch protrusion, soft tissue deformity, mouth opening, complication rate, masticatory function score and treatment satisfaction were compared between the two groups.Results The surgical incision, intraoperative blood loss, operation time and hospitalization time in the treatment group were less than those in the control group (P<0.05). The excellent and good rate of fracture of zygoma and zygomatic arch healing in the treatment group was 95.00%, which was higher than 80.00% in the control group (P<0.05). The bilateral zygomatic arch protrusion and soft tissue deformity in the treatment group were less than those in the control group, and the mouth opening was greater than that in the control group (P<0.05). The incidence of complications in the treatment group was 5.00%, which was lower than 20.00% in the control group (P<0.05). The masticatory function score of the treatment group was lower than that of the control group (P<0.05). The treatment satisfaction of the treatment group was 100.00%, which was higher than 85.00% of the control group (P<0.05).Conclusion The modified scalp semi-coronal incision in the open reduction and internal fixation of fracture of zygoma and zygomatic arch can reduce the intraoperative blood loss, shorten the operation and hospitalization time, improve the excellent and good rate of fracture healing, reduce the bilateral zygomatic arch protrusion and soft tissue deformity, increase the mouth opening, reduce the incidence of complications, promote the recovery of masticatory function, and improve the treatment satisfaction of patients.
