摘要:
目的 探讨缩宫素联合麦角新碱对剖宫产产妇产后出血及母婴结局的影响。方法 选取2021年1月-2023年12月南昌县妇幼保健院接收的60例剖宫产孕妇作为研究对象，根据随机数字表法将其分为对照组与研究组，每组30例。对照组于胎儿娩出后采用缩宫素治疗，研究组在对照组基础上采取马来酸麦角新碱治疗。比较两组产后出血量、凝血功能[D-二聚体、纤维蛋白原（Fib）、凝血酶时间、凝血酶原时间]、不良母婴结局（新生儿窒息、宫内窘迫、低血糖）发生率、不良反应（恶心呕吐、颜面潮红、胸闷憋气、腹痛）发生率。结果 研究组产后2、12、24 h产后出血量均低于对照组（P＜0.05）；研究组治疗后D-二聚体、Fib、凝血酶时间、凝血酶原时间均低于对照组（P＜0.05）；研究组不良母婴结局发生率低于对照组（P＜0.05）；研究组不良反应发生率低于对照组（P＜0.05）。结论 缩宫素联合麦角新碱在剖宫产产妇中的应用效果确切，能够减少产后出血量，改善产妇凝血功能，降低不良母婴结局发生率以及不良反应发生率，值得临床应用。
Abstract:
Objective To investigate the effect of oxytocin combined with ergometrin on postpartum hemorrhage and maternal and infant outcomes in cesarean section. Methods A total of 60 pregnant women with cesarean section received by Nanchang County Maternal and Child Health Hospital from January 2021 to December 2023 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 30 pregnant women in each group. The control group was treated with oxytocin after delivery of the fetus, and the study group was treated with ergometrine maleate on the basis of the control group. The amount of postpartum hemorrhage, coagulation function [D-dimer, fibrinogen (Fib), thrombin time, prothrombin time], incidence of adverse maternal and infant outcomes (neonatal asphyxia, intrauterine distress, hypoglycemia), and incidence of adverse reactions (nausea and vomiting, facial flushing, chest tightness, abdominal pain) were compared between the two groups. Results The amount of postpartum hemorrhage at 2, 12 and 24 h after delivery in the study group was lower than that in the control group (P<0.05). After treatment, D-dimer, Fib, thrombin time and prothrombin time in the study group were lower than those in the control group (P<0.05). The incidence of adverse maternal and infant outcomes in the study group was lower than that in the control group (P<0.05). The incidence of adverse reactions in the study group was lower than that in the control group（P<0.05). Conclusion Oxytocin combined with ergometrine has a definite application effect in cesarean section women, which can reduce postpartum hemorrhage, improve maternal coagulation function, reduce the incidence of adverse maternal and infant outcomes and the incidence of adverse reactions. It is worthy of clinical application.
