摘要:
目的 探讨麦角新碱联合缩宫素在瘢痕子宫剖宫产产后出血患者中的应用效果。方法 选取2022年1月-2024年6月于遂川县妇幼保健院行剖宫产的60例瘢痕子宫再妊娠产妇作为研究对象，按随机数字表法将其分为对照组和研究组，每组30例。对照组给予缩宫素治疗，研究组给予缩宫素注射同时加用马来酸麦角新碱治疗。比较两组临床疗效、出血情况（产后2 h出血量、产后24 h出血量、产后出血率）、血生化水平[红细胞计数（RBC）、红细胞比容（HT）、血红蛋白（Hb）]、凝血指标情况[凝血酶时间（TT）、血浆纤维蛋白原（FIB）、血浆凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）]。结果 研究组治疗总有效率为96.67%，高于对照组的73.33%（P＜0.05）；研究组产后2 h出血量、产后24 h出血量及产后出血率均低于对照组（P＜0.05）；两组产后24 h的RBC、HT、Hb水平均低于产前，而研究组RBC、HT、Hb水平均高于对照组（P＜0.05）；两组产后24 h的FIB水平均高于剖宫产前，而PT、APTT、TT水平均低于剖宫产前，且研究组FIB水平高于对照组，而PT、APTT、TT水平均低于对照组（P＜0.05）。结论 麦角新碱联合缩宫素在瘢痕子宫剖宫产产后出血患者中的效果确切，能够减少产后出血量及产后出血发生率，改善血生化水平，提高凝血功能，值得临床应用。
Abstract:
Objective To explore the application effect of ergometrine combined with oxytocin in patients with postpartum hemorrhage after cesarean section of scarred uterus. Methods From January 2022 to June 2024, 60 pregnant women with scarred uterus who underwent cesarean section in Suichuan County Maternal and Child Health Hospital were selected as the research objects. They were divided into control group and study group according to the random number table method, with 30 pregnant women in each group. The control group was treated with oxytocin, and the study group was treated with oxytocin injection combined with ergonovine maleate. The clinical efficacy, bleeding (the volume of 2 h postpartum hemorrhage, the volume of 24 h postpartum hemorrhage, postpartum hemorrhage rate), blood biochemical level [red blood cell count (RBC), hematocrit (HT), hemoglobin (Hb)], coagulation index [thrombin time (TT), plasma fibrinogen (FIB), plasma prothrombin time (PT), activated partial thromboplastin time (APTT)] were compared between the two groups. Results The total effective rate of treatment in the study group was 96.67%, which was higher than 73.33% in the control group (P<0.05). The volume of 2 h postpartum hemorrhage, the volume of 24 h postpartum hemorrhage and the postpartum hemorrhage rate in the study group were lower than those in the control group (P<0.05). The levels of RBC, HCT and Hb in the two groups at 24 h after delivery were lower than those before delivery, while the levels of RBC, HCT and Hb in the study group were higher than those in the control group (P<0.05). The FIB level at 24 h after delivery in the two groups was higher than that before cesarean section, while the PT, APTT and TT levels were lower than those before cesarean section, and the FIB level in the study group was higher than that in the control group, while the PT, APTT and TT levels were lower than those in the control group (P<0.05). Conclusion Ergometrine combined with oxytocin has a definite effect in patients with postpartum hemorrhage after cesarean section of scarred uterus. It can reduce the volume of postpartum hemorrhage and the incidence of postpartum hemorrhage, improve blood biochemical level and coagulation function, which is worthy of clinical application.
