摘要:
目的 探讨无创呼吸机联合氧气驱动雾化吸入治疗慢阻肺的临床效果及对患者肺功能指标的影响。方法 选取2021年1月-2024年8月玉山县黄家驷医院/玉山县人民医院收治的慢阻肺患者64例为研究对象,根据随机数字表法分为对照组与研究组,各32例。对照组使用无创呼吸机治疗,研究组在对照组基础上使用氧气驱动进行雾化治疗。比较两组治疗效果、症状(咳嗽、呼吸困难、咳痰)缓解时间、肺功能[用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC、肺总量(TLC)、残气量(RV)、RV/TLC、峰值呼气流速(PEF)]、动脉血气指标[心率(HR)、呼吸频率(RR)、血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、血氧饱和度(SaO2)]。结果 研究组治疗总有效率高于对照组(P<0.05);研究组症状缓解时间均短于对照组(P<0.05);治疗后,研究组FEV1、FVC、FEV1/FVC、PEF水平均高于对照组,而TLC、RV、RV/TLC水平均低于对照组(P<0.05);研究组HR、RR、PaCO2水平均低于对照组,而PaO2、SaO2水平均高于对照组(P<0.05)。结论 无创呼吸机联合氧气驱动雾化吸入治疗慢阻肺的临床效果确切,能够加快患者症状缓解,改善患者肺功能以及血气指标水平,值得临床应用。
Abstract:
Objective To investigate the clinical effect of noninvasive ventilator combined with oxygen-driven atomizing inhalation in the treatment of chronic obstructive pulmonary disease and its influence on pulmonary function indexes. Methods A total of 64 patients with chronic obstructive pulmonary disease admitted to Huangjiasi Hospital/Yushan County People′s Hospital from January 2021 to August 2024 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 32 patients in each group. The control group was treated with noninvasive ventilator, and the study group was treated with oxygen-driven atomizing inhalation on the basis of the control group. The therapeutic effect, remission time of symptoms (cough, dyspnea, expectoration), pulmonary function [forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC, total lung capacity (TLC), residual volume (RV), RV/TLC, peak expiratory flow rate (PEF)], arterial blood gas indexes [heart rate (HR), respiratory rate (RR), partial pressure of carbon dioxide (PaCO2), partial pressure of oxygen (PaO2), oxygen saturation (SaO2)] were compared between the two groups. Results The total effective rate of treatment in the study group was higher than that in the control group (P<0.05). The symptom relief time of the study group was shorter than that of the control group (P<0.05). After treatment, the levels of FEV1, FVC, FEV1/FVC and PEF in the study group were higher than those in the control group, while the levels of TLC, RV and RV/TLC were lower than those in the control group (P<0.05). The levels of HR, RR and PaCO2 in the study group were lower than those in the control group, while the levels of PaO2 and SaO2 were higher than those in the control group (P<0.05). Conclusion Noninvasive ventilator combined with oxygen-driven atomizing inhalation has a definite clinical effect in the treatment of chronic obstructive pulmonary disease, which can accelerate the relief of symptoms and improve the pulmonary function and blood gas index of patients. It is worthy of clinical application.
