摘要:
目的 探讨不同高压氧治疗时间窗对脑梗死患者脑能量代谢及血流动力学的影响。方法 选取2021年6月-2024年5月天津北大医疗海洋石油医院收治的80例脑梗死患者作为研究对象,按照随机数字表法将其分为对照组和研究组,每组40例。研究组于发病12 h内施以高压氧治疗。对照组于发病3 d内施以高压氧治疗。比较两组治疗有效率、肢体功能、脑能量代谢水平(血糖、乳酸、血氧饱和度)、脑血流动力学指标[双侧大脑中动脉收缩期峰值速度(PSV)、舒张末期速度(EDV)、平均血流速度(Vm)及搏动指数(PI)]。结果 研究组治疗总有效率高于对照组(P<0.05)。治疗后,研究组肢体功能优于对照组(P<0.05)。研究组血糖、乳酸水平均低于对照组,而血氧饱和度高于对照组(P<0.05)。研究组PSV、EDV、Vm水平均高于对照组,而PI水平均低于对照组(P<0.05)。结论 发病12 h内施以高压氧治疗在脑梗死患者中的应用效果确切,能够改善患者肢体功能以及脑能量代谢水平,提高患者脑血流速度。
Abstract:
Objective To investigate the effects of different time windows of hyperbaric oxygen therapy on brain energy metabolism and hemodynamics in patients with cerebral infarction. Methods From June 2021 to May 2024, 80 patients with cerebral infarction admitted to Tianjin PKUCare CNOOC Hospital were selected as the research objects. Using the random number table method, the patients were assigned to control group and study group, with 40 patients in each group. The study group was treated with hyperbaric oxygen within 12 h of onset. The control group was treated with hyperbaric oxygen within 3 days of onset. The effective rate of treatment, limb function, brain energy metabolism level (blood glucose, lactic acid, blood oxygen saturation), cerebral hemodynamic indexes [bilateral middle cerebral artery peak systolic velocity (PSV), end diastolic velocity (EDV), mean blood flow velocity (Vm) and pulsatility index (PI)] were compared between the two groups. Results The total effective rate of treatment in the study group was higher than that in the control group (P<0.05). After treatment, the limb function of the study group was better than that of the control group (P<0.05). The levels of blood glucose and lactic acid in the study group were lower than those in the control group, while the oxygen saturation was higher than that in the control group (P<0.05). The levels of PSV, EDV and Vm in the study group were higher than those in the control group, while the level of PI were lower than that in the control group (P<0.05). Conclusion The application of hyperbaric oxygen therapy within 12 h of onset in patients with cerebral infarction is effective, which can improve their limb function and brain energy metabolism level, as well as improve the cerebral blood flow velocity.
