摘要:
目的 探究2型糖尿病(T2DM)周围神经病变(DPN)的危险因素,为疾病的临床防治提供参考依据。方法 选取2023年9月-2024年3月湖北省中西医结合医院内分泌科收治的209例T2DM患者作为研究对象,将合并DPN的患者设为B组(n=134),未合并DPN的患者设为A组(n=75),比较两组患者的脉压差、FBG、PFG、HbA1c、LDL-C、TC、PLT、Hb、FT3、FT4、TSH、UA水平及患者的性别、年龄、糖尿病病程、糖尿病慢性并发症情况。结果 209例T2DM患者中合并DPN的患者134例,DPN的发病率为64.11%。与未合并DPN的患者相比,合并DPN的患者年龄更大,病程更长,PFG、HbA1c、LDL-C及FT4水平更高,TSH水平更低,同时患有周围血管病变(PVD)及视网膜病变(DR)的比例更高,差异均有统计学意义(P<0.05)。二元Logistic回归分析提示:年龄、糖尿病病程、HbA1c、LDL-C、TSH、FT4、合并DR为DPN发生的独立影响因素(P<0.05)。结论 临床中病程较长的T2DM患者需积极监测血糖、血脂及甲状腺激素水平,早期防治DR,以期达到预防及控制DPN发生与发展的目的。
Abstract:
Objective To explore the risk factors of diabetic peripheral neuropathy (DPN) in type 2 diabetes mellitus (T2DM), and to provide reference for the clinical prevention and treatment of the disease. Methods A total of 209 patients with T2DM admitted to the Department of Endocrinology, Hubei Provincial Hospital of Integrated Traditional Chinese and Western Medicine from September 2023 to March 2024 were selected as the research objects. The patients with DPN were set as group B (n=134), and the patients without DPN were set as group A (n=75). The pulse pressure difference, FBG, PFG, HbA1c, LDL-C, TC, PLT, Hb, FT3, FT4, TSH, UA levels, gender, age, duration of diabetes, and chronic complications of diabetes mellitus were compared between the two groups. Results There were 134 patients with DPN in 209 patients with T2DM, and the incidence of DPN was 64.11%. Compared with patients without DPN, patients with DPN were older, had longer duration of disease, higher levels of PFG, HbA1c, LDL-C and FT4, lower TSH level, and had higher proportion of peripheral vascular disease (PVD) and retinopathy (DR), the differences were statistically significant (P<0.05). Binary logistic regression analysis showed that age, duration of diabetes mellitus, HbA1c, LDL-C, TSH, FT4, and DR were independent influencing factors for DPN (P<0.05). Conclusion In clinical practice, patients with T2DM with a long duration of disease need to actively monitor blood glucose, blood lipids and thyroid hormone levels, and early prevention and treatment of DR, in order to achieve the purpose of preventing and controlling the occurrence and development of DPN.
