摘要:
目的 探究多层螺旋CT和高场强MRI诊断膝关节隐匿性骨折的效果。方法 选取2021年1月-2023年12月弋阳县人民医院接收的疑似膝关节隐匿性骨折患者60例作为研究对象,所有患者均接受高场强MRI检查及多层螺旋CT检查。以最终手术诊断或关节镜检查结果作为金标准,分析高场强MRI检查与多层螺旋CT检查的结果,比较两种检查方式在膝关节隐性骨折中的诊断效能及膝关节影像特征。结果 金标准结果显示,阳性患者37例,阴性患者23例;高场强MRI检查结果显示,阳性36例,阴性24例;多层螺旋CT检查结果显示,阳性26例,阴性34例。高场强MRI检查在膝关节隐性骨折中的敏感度、准确度、阴性预测值高于多层螺旋CT检查(P<0.05)。高场强MRI检查中各膝关节影像特征检出率高于多层螺旋CT,差异有统计学意义(P<0.05)。结论 与多层螺旋CT比较,高场强MRI诊断膝关节隐匿性骨折的效果更为确切,其敏感度、准确度、阴性预测值更高,可为临床治疗方案的选择提供更科学、可靠的依据。
Abstract:
Objective To explore the effect of multi-slice spiral CT and high-field MRI in the diagnosis of occult fracture of knee joint. Methods A total of 60 patients with suspected occult fracture of knee joint received by Yiyang County People′s Hospital from January 2021 to December 2023 were selected as the research objects. All patients underwent high-field MRI examination and multi-slice spiral CT examination. Taking the final surgical diagnosis or arthroscopic examination results as the gold standard, the results of high-field MRI examination and multi-slice spiral CT examination were analyzed, and the diagnostic efficacy and knee joint imaging characteristics of the two examination methods in the occult fracture of the knee joint were compared. Results The results of gold standard showed that there were 37 positive patients and 23 negative patients. The results of high-field MRI showed that 36 cases were positive and 24 cases were negative. The results of multi-slice spiral CT examination showed that 26 cases were positive and 34 cases were negative. The sensitivity, accuracy and negative predictive value of high-field MRI in occult fracture of knee joint were higher than those of multi-slice spiral CT (P<0.05). The detection rate of knee joint imaging features in high-field MRI was higher than that in multi-slice spiral CT, and the difference was statistically significant (P<0.05). Conclusion Compared with multi-slice spiral CT, high-field MRI is more effective in the diagnosis of occult fracture of knee joint, and its sensitivity, accuracy and negative predictive value are higher, which can provide a more scientific and reliable basis for the choice of clinical treatment plan.
