摘要:
目的 研究血清糖类抗原125（CA125）、人附睾分泌蛋白4（HE4）及卵巢癌风险预测模型（ROMA）对于卵巢肿瘤良恶性诊断中的应用价值。方法 回顾性分析2018年1月~12月我院收治的经手术病理确诊的卵巢肿瘤患者124例，其中47例为卵巢癌患者设为卵巢癌组，77例卵巢良性肿瘤患者设为卵巢良性肿瘤组，通过卵巢癌风险预测模型分析软件计算得出ROMA指数，比较两组血清CA125、HE4及ROMA指数，分析血清CA125、HE4及ROMA指数对卵巢恶性肿瘤的诊断效能，以卵巢良性肿瘤组为对照绘成受试者工作特征（ROC）曲线并计算曲线下面积（AUC）。结果 卵巢癌组血清CA125、HE4及ROMA指数均高于卵巢良性肿瘤组，差异具有统计学意义（P＜0.05），ROMA指数的敏感度高于HE4、特异度高于CA125，且阳性预测值及阴性预测值均优于HE4或CA125单一检测（P＜0.05），血清CA125、HE4和ROMA指数的ROC-AUC分别是0.916、0.869、0.859。结论 ROMA指数相对于血清CA125及HE4，在卵巢肿瘤的良恶性鉴别中临床应用价值更高。
Abstract:
Objective To study the application value of serum carbohydrate antigen 125 (CA125), human epididymal secretory protein 4 (HE4) and ovarian cancer risk prediction model (ROMA) for the diagnosis of benign and malignant ovarian tumors.Methods A retrospective analysis of 124 patients with ovarian tumors diagnosed by surgery and pathology admitted in our hospital from January to December 2018, of which 47 were ovarian cancer patients were set as ovarian cancer group, 77 benign ovarian tumor patients were set as benign ovarian tumor group The ROMA index was calculated by ovarian cancer risk prediction model analysis software. The serum CA125, HE4 and ROMA indexes were compared between the two groups. The diagnostic efficacy of serum CA125, HE4 and ROMA indexes on ovarian malignant tumors was analyzed. Create a receiver operating characteristic (ROC) curve and calculate the area under the curve (AUC).Results The serum CA125, HE4 and ROMA indexes of ovarian cancer group were higher than those of benign ovarian tumor group, the difference was statistically significant(P<0.05). The sensitivity of ROMA index was higher than HE4, the specificity was higher than CA125, and the positive predictive value and negative The predicted values were better than the single detection of HE4 or CA125(P<0.05).The ROC-AUC of serum CA125, HE4 and ROMA indexes were 0.916, 0.869, and 0.859, respectively. Conclusion Compared with serum CA125 and HE4, ROMA index has higher clinical application value in identifying benign and malignant ovarian tumors.
