摘要:
目的 探究血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平在急性呼吸窘迫综合征(ARDS)预测中的价值。方法 回顾性分析2018年1月~2019年8月我院收治的67例ARDS患者临床资料,根据病情严重程度将其分为轻度组(21例)、中度组(22例)、重度组(24例),比较三组血清TNF-α、IL-6、IL-8水平,并用ROC曲线评价其对ARDS患者病情的预测价值。结果 重度组血清TNF-α、IL-6、IL-8水平高于中度组、轻度组,中度组血清TNF-α、IL-6、IL-8水平高于轻度组,差异有统计学意义(P<0.05);血清TNF-α、IL-6、IL-8联合检测敏感度与特异度高于TNF-α、IL-6、IL-8单一检测。结论 血清TNF-α、IL-6、IL-8水平均可早期预测ARDS病情进展,其联合检测敏感度与特异度均较高,效果更为理想。
Abstract:
Objective To investigate the application of serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and interleukin-8 (IL-8) levels in the prediction and evaluation of acute respiratory distress syndrome (ARDS) value. Methods The clinical data of 67 patients with ARDS treated in our hospital from January 2018 to August 2019 were retrospectively analyzed. They were divided into mild group (21 cases), moderate group (22 cases), and severe group (24 cases),according to the severity of the disease,the serum levels of TNF-α, IL-6 and IL-8 were compared between the three groups, and their predictive value for the condition of ARDS patients was evaluated by using the ROC curve. Results Serum levels of TNF-α, IL-6, and IL-8 in the severe group were higher than those in the moderate and mild groups, and the serum levels of TNF-α, IL-6, and IL-8 in the moderate group were higher than those in the mild group,the difference was statistically significant (P<0.05);The sensitivity and specificity of combined detection of TNF-α, IL-6 and IL-8 were higher than that of single detection.Conclusion Serum levels of TNF-α, IL-6, and IL-8 can all predict the progression of ARDS at an early stage, and the combined detection sensitivity and specificity are higher, and the effect is more ideal.
