摘要:
目的 探讨影响骨科手术切口感染危险因素,并提出相关预防措施。方法 选择2016年1月~2019年12月我院收治的骨科手术患者5442例,根据感染情况分为感染组和未感染组,比较两组临床资料,并采用二分类Logistic逐步回归分析手术切口感染的相关危险因素。结果 5442例骨科手术患者,术后感染80例,感染发生率1.47%。两组不同年龄、手术时机、创伤类型、切口类型、手术部位、手术时间、术前合并感染、合并基础疾病及住院时间比较,差异有统计学意义(P<0.05);两组不同性别、麻醉方式比较,差异无统计学差异(P>0.05)。二分类Logistic逐步回归分析显示,年龄、创伤类型、切口类型、术前合并感染、合并基础疾病、住院时间是手术切口感染的影响因素。结论 骨科手术切口感染受年龄、创伤类型、切口类型、术前合并感染、合并基础疾病及住院时间影响,临床中应加强手术室管理,严格无菌操作,重视老年患者,缩短手术时间,减少组织破坏,有效控制感染风险。
Abstract:
Objective To explore the risk factors that affect the infection of orthopedic surgical incisions and to propose relevant preventive measures. Methods A total of 5,442 orthopedic surgery patients admitted to our hospital from January 2016 to December 2019 were selected. They were divided into infected group and non-infected group according to their infection status. The clinical data of the two groups were compared, and binary Logistic regression was used to analyze surgical wound infection related risk factors.Results In 5442 patients undergoing orthopedic surgery, 80 cases were infected after surgery, and the infection rate was 1.47%. There was a statistically significant difference between the two groups of different ages, surgical timing, trauma types, incision types, surgical sites, surgical time, preoperative infections, combined underlying diseases, and length of hospital stay (P<0.05); the two groups had different genders and anesthesia,there was no statistical difference between the methods (P>0.05). The binary Logistic stepwise regression analysis showed that age, wound type, incision type, preoperative co-infection, co-infection with underlying disease, and length of stay were the influencing factors of surgical incision infection.Conclusion The infection of orthopedic surgical incisions was affected by age, trauma type, incision type, preoperative co-infection, combined with underlying diseases, and length of hospitalization. In clinical practice, the management of the operating room should be strengthened, aseptic operation should be strictly enforced, elderly patients should be emphasized, surgical time should be shortened, and tissue should be reduced Destroy, effectively control the risk of infection.
