摘要:
目的 探讨微波消融术结合聚桂醇硬化治疗甲状腺囊性结节的治疗效果,为规范甲状腺囊性结节的治疗提供相应依据。方法 选择2017年3月~2019年3月我院诊治的35例甲状腺囊性结节作为研究对象,均实施微波消融+聚桂醇硬化术,观察术后甲状腺结节灭活情况、体积改变情况、甲状腺血流、颈部淋巴结情况、治疗总有效率、甲状腺功能及并发症发生情况。结果 术后患者甲状腺无血流灌注体积与术前原结节血流灌注体积比较,差异无统计学意义(P>0.05);术后即时、1、3、6个月甲状腺结节体积均低于术前结节体积,差异有统计学意义(P<0.05);术后即时、1、3、6个月治疗总有效率分别为60.53%、76.32%、97.37%、100.00%;治疗后38枚甲状腺结节内部均无血流信号,1枚结节术后周边见点状血流信号;术后1天患者T4、FT4、Tg、TgAb、TPoAb、TSHAb、TSH与术前比较,差异有统计学意义(P<0.05),术后1月与术前比较,差异无统计学意义(P>0.05);治疗前后患者T3、FT3、Tg水平比较,差异无统计学意义(P>0.05);治疗期间所有患者均未出现并发症。结论 微波消融术结合聚桂醇硬化治疗甲状腺囊性结节疗效确切,可有效灭活甲状腺,阻断甲状腺血流,同时不会对甲状腺功能造成影响,安全可靠。
Abstract:
Objective To investigate the therapeutic effect of microwave ablation combined with cinnamyl alcohol sclerotherapy in the treatment of thyroid cystic nodules, and to provide corresponding basis for standardizing the treatment of thyroid cystic nodules.Methods 35 cases of thyroid cystic nodules diagnosed and treated in our hospital from March 2017 to March 2019 were selected as the research objects.Microwave ablation + cinnamyl alcohol sclerotherapy were performed, thyroid nodules inactivation, volume changes, thyroid blood flow, cervical lymph nodes, total effective rate of treatment, thyroid function, and complications.Results There was no significant difference between the postoperative thyroid no blood perfusion volume and the preoperative original nodule blood perfusion volume (P>0.05);The volume of thyroid nodules immediately, 1, 3, and 6 months after operation was lower than that of preoperative nodules, the difference was statistically significant (P<0.05);The total effective rates of immediate, 1, 3, and 6 months treatment were 60.53%, 76.32%, 97.37%, and 100.00%, respectively;After treatment, there was no blood flow signal in 38 thyroid nodules.Spot-like blood flow signals were seen around one nodule after surgery; the patients T4, FT4, Tg, TgAb, TPoAb, TSHAb, and TSH were compared with those before surgery on the first day after surgery,the difference was statistically significant (P<0.05).There was no significant difference between 1 month after operation and before operation (P>0.05);There was no significant difference in the T3, FT3, and Tg levels of patients before and after treatment (P>0.05);During the treatment, all patients had no complications.Conclusion Microwave ablation combined with cinnamyl alcohol sclerotherapy is effective in the treatment of thyroid cystic nodules. It can effectively inactivate the thyroid and block the blood flow of the thyroid.At the same time, it will not affect thyroid function and is safe and reliable.
