摘要:
目的 比较对拟行手术治疗的疝气患儿应用七氟烷吸入麻醉与异丙酚静脉麻醉的效果,评估其对患儿血流动力学、应激反应的影响。方法 选取2019年10月-2020年10月在我院接受手术治疗的疝气患儿78例,根据随机数字表法分为吸入组和静脉组,各39例。吸入组行七氟烷吸入麻醉,静脉组给予异丙酚静脉麻醉,比较麻醉期两组血流动力学[收缩压(SBP)、舒张压(DBP)及心率(HR)]、应激反应[肾素(R)、去甲肾上腺素(NE)及肾上腺素(E)]及术后意识恢复情况(意识恢复时间、定向力恢复时间、首次排气时间及清醒时间)。结果 T1、T2、T3时,两组NE、E、R、SBP、DBP及HR水平均较T0时上升,吸入组的上升幅度更小,差异有统计学意义(P<0.05);吸入组的意识恢复时间、定向力恢复时间、首次排气时间及清醒时间均比静脉组更短,差异有统计学意义(P<0.05);吸入组诱导期及苏醒期的PAED评分均比静脉组更低,差异有统计学意义(P<0.05)。结论 对实施手术治疗的疝气患儿应用七氟烷吸入麻醉的效果比异丙酚静脉麻醉更为理想,且对患儿血流动力学影响较小,可减轻机体应激反应,缩短患儿意识恢复时间。
Abstract:
Objective To compare the effects of sevoflurane inhalation anesthesia and propofol intravenous anesthesia on children with hernia who are to be treated for surgical treatment, and to evaluate the effects on the children’s hemodynamics and stress response.Methods A total of 78 children with hernia who underwent surgical treatment in our hospital from October 2019 to October 2020 were selected and divided into inhalation group and intravenous group according to the random number table method, with 39 cases in each group.The inhalation group was given sevoflurane inhalation anesthesia, and the intravenous group was given propofol intravenous anesthesia.The hemodynamics [systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR)], stress response [renin (R), norepinephrine (NE) and epinephrine (E)] were compared between the two groups during anesthesia and postoperative consciousness recovery (consciousness recovery time, orientation recovery time, first exhaust time and awake time).Results At T1, T2, and T3, the levels of NE, E, R, SBP, DBP and HR in the two groups increased compared with those at T0. The increase in the inhalation group was smaller,the difference was statistically significant (P<0.05);The recovery time of consciousness, the recovery time of orientation, the time of first exhaust and the time of waking up in the inhalation group were shorter than those in the intravenous group,the difference was statistically significant (P<0.05);The PAED scores of the inhalation group during the induction and recovery phases were lower than those of the intravenous group,the difference was statistically significant (P<0.05).Conclusion The effect of sevoflurane inhalation anesthesia for children with hernia undergoing surgical treatment is more ideal than propofol intravenous anesthesia, with less impact on the hemodynamics of the children.It can reduce the body’s stress response and shorten the recovery time of children’s consciousness.
