摘要:
目的 对2011-2020年广东省21个地市卫生资源配置效率进行测算,分析配置效率情况并提出建议。方法 从2020年《广东省卫生健康统计信息简本》中获取2011-2020年广东省各地市医疗机构数、技术人员数、床位数、诊疗人次以及出院人次等数据作为投入指标和产出指标,通过软件DEAP2.1使用DEA-BCC模型和Malmquist指数模型对2011-2020年广东省21个地市卫生资源投入指标和产出指标进行静态和动态分析。结果 静态分析方面,2011-2020年广东省卫生资源配置综合效率均值为0.860,佛山、中山综合效率始终为数据包络分析(DEA)有效;纯技术效率均值为0.933,其中8市始终为DEA有效;规模效率均值为0.924,部分城市出现规模效率水平低下的问题;整体效率较低,处于缓慢上升态势,卫生资源配置存在地理空间差异。动态分析方面,Malmquist全要素生产率指数近十年的均值为0.967,韶关、湛江、茂名Malmquist指数大于1,占比14.3%;2011-2012年,2018-2019年Malmquist指数大于1,整体处于下降趋势,主要受到技术变动的影响。结论 广东省卫生资源配置效率存在地区不平衡的现象,推动技术创新,优化管理,改善制度,健康公平是今后工作的重点内容。
Abstract:
Objective To calculate the allocation efficiency of health resources in 21 prefectures and cities of Guangdong Province from 2011 to 2020, analyze the results of allocation efficiency and make suggestions.Methods The data of the number of medical institutions, the number of technicians, the number of beds, the number of patients and the number of discharged patients in each city of Guangdong Province from 2011 to 2020 were obtained from the “Guangdong Provincial Health Statistics Compendium” as input and output indicators. The DEA-BCC model and the Malmquist index model were used to analyze the input and output indicators of health resources in 21 cities of Guangdong Province from 2011 to 2020.Results In terms of static analysis, the average comprehensive efficiency of health resource allocation in Guangdong Province from 2011 to 2020 was 0.860, and the comprehensive efficiency of Foshan and Zhongshan was always effective for data envelopment analysis (DEA). The average pure technical efficiency was 0.933, of which 8 cities were always DEA effective; the average scale efficiency was 0.924, and some cities had low scale efficiency. The overall efficiency was low, in a slow upward trend, the allocation of health resources exist geospatial differences. In terms of dynamic analysis, the average value of Malmquist total factor productivity index in the past ten years was 0.967. The Malmquist index of Shaoguan, Zhanjiang and Maoming was greater than 1, accounting for 14.3%; in 2011-2012 and 2018-2019, the Malmquist index was greater than 1, and the overall trend was downward, mainly affected by technological changes.Conclusion There is still regional imbalance in the allocation efficiency of health resources in Guangdong Province. Promoting technological innovation, management optimization, system improvement and health equity are the key contents of future work.
