摘要:
目的 探索经腹腔镜下卵巢囊肿剥除术与经腹卵巢囊肿剥除术对卵巢囊肿患者术后卵巢储备功能的影响差异。方法 选取2017年11月-2021年1月在我院确诊为卵巢囊肿的患者共60例，采用随机数字表法分为观察组和对照组，每组30例。对照组行经腹卵巢囊肿剥除术，观察组行经腹腔镜下卵巢囊肿剥除术，比较两组手术相关指标（手术时间、术中和术后出血量及住院时间）、术后卵巢储备功能[黄体生成素（LH）、卵泡刺激素（FSH）、雌激素（E2）和抗缪勒氏管激素（AMH）]、血清炎症因子[C反应蛋白（CRP）、白介素6（IL-6）和降钙素原（PCT）]及并发症发生情况。结果 观察组手术时间、术中出血量和住院时间均低于对照组，差异有统计学意义（P＜0.05）；观察组术后出血量略低于对照组，但差异无统计学意义（P＞0.05）；观察组LH、FSH水平低于对照组，E2和AMH水平高于对照组，差异有统计学意义（P＜0.05）；观察组CRP、IL-6、PCT水平低于对照组，差异有统计学意义（P＜0.05）；两组术后并发症发生率比较，差异无统计学意义（P＞0.05）。结论 经腹腔镜下卵巢囊肿剥除术与经腹卵巢囊肿剥除术相比，对卵巢囊肿患者术后卵巢储备功能的影响更小，手术时间短、出血量少，患者炎性因子水平低，并发症少，安全性高，值得临床应用。
Abstract:
Objective To explore the effect of laparoscopic ovarian cystectomy and transabdominal ovarian cystectomy on postoperative ovarian reserve function in patients with ovarian cysts.Methods A total of 60 patients with ovarian cysts diagnosed in our hospital from November 2017 to January 2021 were randomly selected and divided into observation group and control group by random number table method, with 30 cases in each group. The control group underwent transabdominal ovarian cystectomy, and the observation group underwent laparoscopic ovarian cystectomy. The operation-related indicators (operation time, intraoperative and postoperative bleeding volume and hospitalization time), postoperative ovarian reserve function [luteinizing hormone (LH), follicle stimulating hormone (FSH), estrogen (E2) and anti-Mullerian hormone (AMH)], serum inflammatory factors [C-reactive protein (CRP), interleukin 6 (IL-6) and procalcitonin (PCT)] and complications were compared between the two groups.Results The operation time, intraoperative blood loss and hospitalization time of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05). The amount of postoperative bleeding in the observation group was slightly lower than that in the control group, but the difference was not statistically significant (P>0.05). The levels of LH and FSH in the observation group were lower than those in the control group, and the levels of E2 and AMH were higher than those in the control group, the difference was statistically significant (P<0.05). The levels of CRP, IL-6 and PCT in the observation group were lower than those in the control group, and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of postoperative complications between the two groups (P>0.05).Conclusion Compared with transabdominal ovarian cystectomy, laparoscopic ovarian cystectomy has less effect on postoperative ovarian reserve function in patients with ovarian cysts, shorter operation time, less bleeding, lower inflammatory factor level, fewer complications and higher safety, which is worthy of clinical application.
