摘要:
目的 研究针灸联合电脑中频电疗治疗肩周炎的临床疗效及对白细胞介素-1β（IL-1β）、肿瘤坏死因子-α（TNF-α）、关节功能的影响。方法 选取2020年4月-2022年4月吉水县人民医院收治的126例肩周炎患者，采用随机数字表法分为对照组（63例）和观察组（63例）。对照组应用电脑中频电疗，观察组应用针灸联合电脑中频电疗，比较两组临床疗效、中医症候积分、疼痛程度[视觉模拟评分（VAS）]、IL-1β、TNF-α、肩关节功能（肩关节后伸角、Constant-Murley肩关节功能评分）。结果 观察组治疗总有效率为95.24%，高于对照组的84.13%（P＜0.05）。两组治疗后中医症候积分、VAS评分低于治疗前，且观察组低于对照组（P＜0.05）。两组治疗后IL-1β、TNF-α水平低于治疗前，且观察组低于对照组（P＜0.05）。两组治疗后肩关节后伸角大于治疗前、Constant-Murley评分高于治疗前，且观察组肩关节后伸角大于对照组、Constant-Murley评分高于对照组（P＜0.05）。结论 针灸联合电脑中频电疗治疗肩周炎的疗效确切，可缓解患者症状，减轻肩部疼痛，下调IL-1β、TNF-α水平，改善肩关节功能。
Abstract:
Objective To study the clinical efficacy of acupuncture combined with computer intermediate frequency electrotherapy in the treatment of periarthritis humeroscapularis and its effect on interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α) and joint function.Methods A total of 126 patients with periarthritis humeroscapularis admitted to Jishui County People’s Hospital from April 2020 to April 2022 were selected and divided into control group (63 patients) and observation group (63 patients) by random number table method. The control group was treated with computer intermediate frequency electrotherapy, and the observation group was treated with acupuncture combined with computer intermediate frequency electrotherapy. The clinical efficacy, TCM symptom score, pain degree [Visual Analogue Scale (VAS)], IL-1β, TNF-α, shoulder joint function (shoulder joint extension angle, Constant-Murley shoulder joint function score) were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.24%, which was higher than 84.13% in the control group (P<0.05). After treatment, the scores of TCM symptoms and VAS in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). After treatment, the levels of IL-1β and TNF-α in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). After treatment, the shoulder extension angle of the two groups was greater than that before treatment, and the Constant-Murley score was higher than that before treatment, while the shoulder extension angle of the observation group was greater than that of the control group, and the Constant-Murley score was higher than that of the control group (P<0.05).Conclusion Acupuncture combined with computer intermediate frequency electrotherapy is effective in the treatment of periarthritis of shoulder, which can relieve the symptoms of patients, reduce shoulder pain, down-regulate the levels of IL-1β and TNF-α, and improve the function of shoulder joint.
