摘要:
目的 探讨糖尿病患者住院费用结构变动的影响因素，为优化住院费用结构和科学有效控费提供依据。方法 以2017-2021年榆林市第一医院住院的糖尿病患者为研究对象，从病案信息系统中提取患者住院费用数据。利用灰色关联度分析次均住院费用与其各个分项费用的关联程度。利用结构变动度分析各个分项费用的变动程度、变动方向以及对住院费用的影响程度。结果 2017-2021年各项费用的构成占比最大的是药品费（38.33%），关联度排在前3位的是药品费（0.969）、检验费（0.963）、治疗费（0.811）；2017-2018年结构变动度（11.81%）最大，结构贡献率排在前3位的是检验费（48.68%）、药品费（39.35%）、床位费（8.07%），三者累计贡献率达96.10%。结论 药品费和检验费是影响次均住院费用的主要内部影响因素，医疗服务型收入占比较低。应进一步优化费用结构，提高医疗技术水平，规范临床路径，建立合理有效的控费机制。
Abstract:
Objective To explore the influencing factors of the structural variation of hospitalization expenses in diabetic mellitus patients, and to provide a basis for optimizing the structure of hospitalization expenses and scientific and effective cost control.Methods Taking diabetic patients hospitalized in the First Hospital of Yulin from 2017 to 2021 as the research object, the hospitalization cost data of patients were extracted from the medical record information system. The grey correlation degree was used to analyze the correlation degree between the average hospitalization cost and each item cost. The degree of structural variation was used to analyze the degree of change, the direction of change and the degree of influence on hospitalization expenses.Results From 2017 to 2021, the largest proportion of each cost was drug cost (38.33%), and the top three of correlation degree were drug cost (0.969), examination cost (0.963) and treatment cost (0.811). From 2017 to 2018, the degree of structural variation (11.81%) was the largest, and the top three structural contribution rates were examination cost (48.68%), drug cost (39.35%), and bed cost (8.07%). The cumulative contribution rate of the three was 96.10%.Conclusion Drug cost and examination cost are the main internal factors affecting the average hospitalization cost, and the proportion of medical service income is relatively low. It is necessary to further optimize the cost structure, improve the level of medical technology, standardize the clinical pathway, and establish a reasonable and effective cost control mechanism.
