摘要:
目的 研究不同血清乙肝病毒DNA水平对乙肝患者肝功能指标及乙肝标志物的影响。方法 选取2000年1月-2022年1月天津市第二人民医院诊治的356例乙肝患者作为观察组,依据病毒载量水平将观察组分为A组(<5.0×102 kU/L)、B组(5.0×102~1.0×104 kU/L)、C组(1.0×104~1.0×106 kU/L)、D组(1.0×106~1.0×108 kU/L),并选取同期在天津市第二人民医院体检健康者356例作为对照组,比较各组肝功能指标 [天冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)]水平,并分析观察组中不同乙肝标志物(HBV-M)模式乙肝患者HBV-DNA阳性检出率和DNA含量,另采用Pearson相关性分析HBV-M定量与HBV-DNA载量、肝功能指标的关系。结果 观察组AST、ALT水平均高于对照组(P<0.05);不同HBV-DNA病毒载量乙肝患者AST、ALT水平比较,差异有统计学意义(P<0.05),且D组>C组>B组>A组;不同HBV-M模式乙肝患者HBV-DNA阳性检出率、DNA含量比较,差异有统计学意义(P<0.05);Pearson相关性分析显示,HBsAg阳性与AST、ALT无相关性(P>0.05),与HBV-DNA载量呈正相关(P<0.05);HBeAg、HBeAb阳性与HBV-DNA载量、AST呈正相关(P<0.05),与ALT无相关性(P>0.05);HBcAb阳性与HBV-DNA载量、ALT、AST均呈正相关(P<0.05)。结论 不同血清乙肝病毒DNA水平对乙肝患者肝功能指标及乙肝标志物具有一定的影响,肝功能指标会随乙肝病毒DNA的复制不断升高,乙肝标志物定量检测可评估乙肝体内病毒复制情况,两者联合检测有助于及早发现患者肝功能损伤,从而为临床制定治疗方案提供可靠的依据。
Abstract:
Objective To study the effects of different serum hepatitis B virus DNA levels on liver function indexes and hepatitis B markers in hepatitis B patients.Methods A total of 356 patients with hepatitis B diagnosed and treated in Tianjin Second People’s Hospital from January 2000 to January 2022 were selected as the observation group. According to the level of viral load, the observation group was divided into group A (<5.0×102 kU/L), group B (5.0×102-1.0×104 kU/L), group C (1.0×104-1.0×106 kU/L), group D (1.0×106-1.0×108 kU/L), and 356 healthy people in Tianjin Second People’s Hospital during the same period were selected as the control group. The levels of liver function indexes [aspartate aminotransferase (AST), alanine aminotransferase (ALT)] in each group were compared, and the positive detection rate of HBV-DNA and DNA content in hepatitis B patients with different hepatitis B markers (HBV-M) in the observation group were analyzed. Pearson correlation was used to analyze the relationship between HBV-M quantification and HBV-DNA load and liver function indexes.Results The levels of AST and ALT in the observation group were higher than those in the control group (P<0.05). There were significant differences in AST and ALT levels among patients with different HBV-DNA viral loads (P<0.05), and group D>group C>group B>group A. There were statistically significant differences in HBV-DNA positive detection rate and DNA content among hepatitis B patients with different HBV-M models (P<0.05). Pearson correlation analysis showed that HBsAg positive was not correlated with AST and ALT (P>0.05), but positively correlated with HBV-DNA load (P<0.05); HBeAg and HBeAb positive were positively correlated with HBV-DNA load and AST (P<0.05), but not with ALT (P>0.05); HBcAb positive was positively correlated with HBV-DNA load, ALT and AST (P<0.05).Conclusion Different serum hepatitis B virus DNA levels have certain effects on liver function indexes and hepatitis B markers in hepatitis B patients. Liver function indexes will continue to increase with the replication of hepatitis B virus DNA. Quantitative detection of hepatitis B markers can evaluate the replication of hepatitis B virus in vivo. The combined detection of the two is helpful for early detection of liver function damage in patients, so as to provide a reliable basis for clinical treatment.
