摘要:
目的 探讨浙江地区体检人群糖化血红蛋白的水平的分布情况。方法 收集2023年2月-5月本中心15 009例体检人员样本,根据不同性别、年龄分为6个组,分别为≤30岁,31~40岁,41~50岁,51~60岁,61~70岁,>70岁组。采用罗氏cobas c513全自动糖化血红蛋白分析仪,免疫比浊法检测HbA1c水平。分别按不同性别、不同年龄段、HbA1c参考区间,以及2023年糖尿病诊断新标准对样本进行分组,并进行分组统计分析。结果 男性HbA1c水平高于女性(P<0.05)。≤30岁、31~40岁、41~50岁组分别与51~60岁组比较,差异有统计学意义(P<0.05);≤30岁、31~40岁、41~50岁、51~60岁组分别与61~70岁组比较,差异有统计学意义(P<0.05);61~70岁与>70岁组比较,差异无统计学意义(P>0.05)。男性≤30岁、31~40岁、41~50岁组HbA1c水平随年龄增长而升高,41~50岁之后HbA1c水平差别较小;女性HbA1c水平整体随年龄增长而升高;31~40岁、41~50岁、51~60岁组不同性别HbA1c水平分别比较,差异有统计学意义(P<0.05)。根据2023年糖尿病诊断新标准(5.7%~6.4%),5.7%~6.4%有7017例,占46.75%;6.5%~8.0%有1637例,占10.90%;>8.0%有790例,占5.26%。结论 不同性别间随年龄变化的分布趋势有所不同,根据2023年新指南筛查出疑似糖尿病者增多,对糖尿病的早发现早治疗具有重要的临床意义,也是临床决定是否需要调整治疗的重要依据。
Abstract:
Objective To investigate the distribution of glycosylated hemoglobin level in physical examination population in Zhejiang area. Methods From February to May 2023, a sample of 15 009 physical examination personnel in our center was collected. According to different genders and ages, they were divided into 6 groups: ≤30 years old group, 31-40 years old group, 41-50 years old group, 51-60 years old group, 61-70 years old group, >70 years old group. The level of HbA1c was detected by Roche cobas c513 automatic glycosylated hemoglobin analyzer and immunoturbidimetry. The samples were grouped according to different genders, different age groups, HbA1c reference intervals, and the new diagnostic criteria for diabetes in 2023, and statistical analysis was performed.Results The level of HbA1c in males was higher than that in females (P<0.05). There were statistically significant differences in HbA1c level between ≤30 years old group, 31-40 years old group, 41-50 years old group and 51-60 years old group, respectively (P<0.05). There were significant differences in HbA1c level between ≤30 years old group, 31-40 years old group, 41-50 years old group, 51-60 years old group and 61-70 years old group, respectively (P<0.05). There was no significant difference in HbA1c level between 61-70 years old group and >70 years old group (P>0.05). The level of HbA1c in male≤ 30 group, 31-40 group and 41-50 group increased with age, and there was little difference in HbA1c level after 41-50 years old. The level of HbA1c in females increased with age, and there were significant differences in HbA1c level between different genders in 31-40 years old, 41-50 years old and 51-60 years old groups (P<0.05). According to the new diagnostic criteria for diabetes mellitus in 2023 (5.7%-6.4%), 5.7%-6.4% had 7017 cases, accounting for 46.75%; there were 1637 cases of 6.5%-8.0%, accounting for 10.90%; there were 790 cases of >8.0%, accounting for 5.26%.Conclusion The distribution trend of change with age is different between different gender. According to the new guidelines in 2023, the number of suspected diabetes mellitusincreased, which has important clinical significance for the early detection and treatment of diabetes, and is also an important basis for clinical decision to decide whether to adjust treatment.
