摘要:
目的 研究微创INTERTAN髓内钉对股骨粗隆间骨折隐性失血、髋关节功能的影响。方法 选取2022年6月-2023年6月在我院诊治的66例股骨粗隆间骨折患者为研究对象，采用随机数字表法分为对照组（n=33）和观察组（n=33），对照组采用股骨近端抗旋髓内钉（PFNA）治疗，观察组采用INTERTAN髓内钉治疗。比较两组手术相关指标、髋关节功能、疼痛评分（VAS）及术后并发症发生率。结果 观察组手术时间、住院时间短于对照组，围手术期失血量大于对照组（P＜0.05）；两组术后3个月Harris 髋关节功能各维度（髋关节活动度、运动、功能性活动）评分均高于术前，且观察组高于对照组（P＜0.05）；两组术后1、3个月VAS评分均低于术前，且观察组低于对照组（P＜0.05）；观察组并发症发生率低于对照组，但差异无统计学意义（P＞0.05）。结论 微创INTERTAN髓内钉治疗股骨粗隆间骨折具有良好的有效性，可促进髋关节功能恢复，减轻患者疼痛，且术后并发症少，是一种可行、有效的治疗方案。
Abstract:
Objective To study the effect of minimally invasive INTERTAN intramedullary nail on hidden blood loss and hip joint function in intertrochanteric fracture of femur.Methods From June 2022 to June 2023, 66 patients with intertrochanteric fracture of femur diagnosed and treated in our hospital were selected as the research objects. They were divided into control group (n=33) and observation group (n=33) by random number table method. The control group was treated with proximal femoral nail antirotation (PFNA), and the observation group was treated with INTERTAN intramedullary nail. The operation related indexes, hip joint function, pain score (VAS) and the incidence of postoperative complications were compared between the two groups.Results The operation time and hospitalization time of the observation group was shorter than that of the control group, and the perioperative blood loss was greater than that of the control group (P<0.05). The scores of Harris hip joint function (hip joint activity, movement, functional activity) in the two groups at 3 months after operation were higher than those before operation, and those in the observation group were higher than those in the control group (P<0.05). The VAS score of the two groups on 1 month and 3 month after operation was lower than that before operation, and that in the observation group was lower than that in the control group (P<0.05). The incidence of complications in the observation group was lower than that in the control group, but the difference was not statistically significant (P>0.05).Conclusion Minimally invasive INTERTAN intramedullary nail is effective in the treatment of intertrochanteric fractures of the femur, which can promote the recovery of hip joint function, reduce the pain of patients, and have fewer postoperative complications. It is a feasible and effective treatment plan.
