摘要:
目的 研究乙肝两对半与病毒DNA(HBV-DNA)联合检验在乙肝诊断中的应用价值。方法 选取2020年1月-2023年1月余干县中医院确诊的60例乙肝患者设为乙肝组,另取同期50名体检健康者设为健康组,两组均给予乙肝两对半[乙肝表面抗原(HBsAg)、乙肝表面抗体(HBsAb)、乙肝e抗原(HBeAg)、乙肝e抗体(HBeAb)、乙肝核心抗体(HBcAb)]与HBV-DNA含量检测,比较乙肝组与健康组乙肝两对半与HBV-DNA测定结果,统计不同HBV-M模式的HBV-DNA测定结果,以综合诊断结果为金标准,分析并比较乙肝两对半、HBV-DNA单独检验及联合检验对乙肝的诊断效能。结果 乙肝组乙肝两对半指标(HBsAg、HBsAb、HBeAg、HBeAb、HBcAb)与HBV-DNA测定结果均高于健康组(P<0.05);不同HBV-M模式下HBV-DNA测定结果比较,差异有统计学意义(P<0.05),其含量由高至低依次为HBsAg+HBeAg+HBcAb>HBsAg+HBeAb+HBcAb>HBsAg+HBcAb>HBsAg+HBsAb+HBeAb>HBsAg+HBeAb>HBsAg;乙肝两对半与HBV-DNA联合检验对乙肝的诊断准确性、敏感度、特异度高于其单一检验(P<0.05)。结论 乙肝两对半与HBV-DNA检测在乙肝诊断中均具有确切参考价值,二者联合检测可发挥更高诊断效能。
Abstract:
Objective To study the application value of combined detection of HBV antigen, antibody and viral DNA (HBV-DNA) in the diagnosis of hepatitis B.Methods A total of 60 patients with hepatitis B diagnosed in Yugan County Hospital of Traditional Chinese Medicine from January 2020 to January 2023 were selected as the hepatitis B group, and 50 healthy people in the same period were selected as the healthy group. Both groups were detected HBV antigen, antibody [hepatitis B surface antigen (HBsAg), hepatitis B surface antibody (HBsAb), hepatitis B e antigen (HBeAg), hepatitis B e antibody (HBeAb), hepatitis B core antibody (HBcAb)] and HBV-DNA content. The results of HBV antigen, antibody and HBV-DNA in the hepatitis B group and the healthy group were compared. The results of HBV-DNA determination in different HBV-M models were statistically analyzed. Taking the comprehensive diagnosis results as the gold standard, the diagnostic efficacy of HBV antigen, antibody, HBV-DNA single test and combined test for hepatitis B was analyzed and compared.Results The results of hepatitis B two half-and-half indicators ( HBsAg, HBsAb, HBeAg, HBeAb, HBcAb) and HBV-DNA determination in the hepatitis B group were higher than those in the healthy group (P<0.05). There were significant differences in the results of HBV-DNA determination under different HBV-M modes (P<0.05), while the content of HBV-DNA from high to low was HBsAg+HBeAg+HBcAb>HBsAg+HBeAb+HBcAb>HBsAg+HBcAb>HBsAg+HBsAb+HBeAb>HBsAg+HBeAb>HBsAg. The diagnostic accuracy, sensitivity and specificity of the combined test of hepatitis B and HBV-DNA were higher than those of the single test (P<0.05).Conclusion The HBV antigen, antibody and HBV-DNA detection have definite reference value in the diagnosis of hepatitis B, and the combined detection of the two can play a higher diagnostic efficiency.
