摘要:
目的 观察不同剂量瑞芬太尼用于无痛胃镜检查的有效性及安全性。方法 选取2020年3月-2023年3月在我院进行无痛胃镜检查的63例患者为研究对象，采用随机数字表法分为A组、B组、C组，各组21例。A采用0.1 μg/kg瑞芬太尼复合丙泊酚，B组采用0.3 μg/kg瑞芬太尼复合丙泊酚，C组采用0.6 μg/kg瑞芬太尼复合丙泊酚，比较三组麻醉效果、麻醉起效时间、定向力恢复时间、苏醒时间、生命体征[平均动脉压（MAP）、心率（HR）、血氧饱和度（SpO2）]、不耐受反应发生率、不良反应发生率。结果 B组、C组麻醉优良率均高于A组（P＜0.05），但B组和C组比较，差异无统计学意义（P＞0.05）；C组、B组麻醉起效时间、定向力恢复时间、苏醒时间均短于A组（P＜0.05），但B组和C组比较，差异无统计学意义（P＞0.05）；三组MAP、HR均低于给药前，且C组低于A组、B组（P＜0.05），A组和B组比较，差异无统计学意义（P＞0.05），三组给药前后SpO2比较，差异无统计学意义（P＞0.05）；C组不耐受反应发生率、不良反应发生率大于A组和B组（P＜0.05），但A组与B组比较，差异无统计学意义（P＞0.05）。结论 不同剂量瑞芬太尼用于无痛胃镜检查有效性及安全性存在一定差异，但是相对而言，0.3 μg/kg瑞芬太尼不仅可实现良好的麻醉效果，而且可降低不良反应和不耐受发生率，维持生命体征相对稳定，保持良好的检查结束苏醒质量。
Abstract:
Objective To observe the efficacy and safety of different doses of remifentanil for painless gastroscopy.Methods A total of 63 patients who underwent painless gastroscopy in our hospital from March 2020 to March 2023 were selected as the research objects. They were divided into group A, group B and group C by random number table method, with 21 patients in each group. The control group was treated with 0.1 μg/kg remifentanil combined with propofol, the group B was treated with 0.3 μg/kg remifentanil combined with propofol, and the group C was treated with 0.6 μg/kg remifentanil combined with propofol. The anesthetic effect, anesthesia onset time, orientation recovery time, recovery time, vital signs [mean arterial pressure (MAP), heart rate (HR), oxygen saturation (SpO2)], the incidence of intolerance and the incidence of adverse reactions were compared among the three groups.Results The excellent and good rate of anesthesia in group B and group C was higher than that in group A (P<0.05), but there was no significant difference between group B and group C (P>0.05). The onset time of anesthesia, recovery time of directional force and recovery time in group C and group B were shorter than those in group A (P<0.05), but there was no significant difference between group B and group C (P>0.05). The MAP and HR of the three groups were lower than those before administration, and those of the group C were lower than those of group A and group B (P<0.05), but there were no significant difference between group A and group B (P>0.05). There was no significant difference in SpO2 between the three groups before and after administration (P>0.05). The incidence of intolerance and adverse reactions in group C was higher than that in group A and group B (P<0.05), but there was no significant difference between group A and group B (P>0.05).Conclusion There are some differences in the efficacy and safety of different doses of remifentanil for painless gastroscopy, but relatively speaking, 0.3 μg/kg remifentanil can not only achieve good anesthetic effect, but also reduce the incidence of adverse reactions and intolerance, maintain relatively stable vital signs, and maintain good quality of recovery after examination.
