摘要:
目的 分析不同剂量阿托伐他汀治疗心梗后无症状心衰的疗效。方法 选择2018年1月~2019年1月在我院诊治的120例心梗后无症状心衰患者，采用随机数字表法分为对照组和观察组，各60例，对照组给予20 mg/d阿托伐他汀治疗，观察组给予40 mg/d阿托伐他汀治疗，比较两组临床治疗总有效率、治疗前后心率（HR）、左室射血分数（LVEF）、心室收缩末期容积（LVESV）、血浆BNP、超敏C-反应蛋白（hs-CRP）水平。结果 观察组治疗总有效率为93.33%，高于对照组的81.67%，差异有统计学意义（P＜0.05）；治疗后两组LVEF均高于治疗前，且观察组高于对照组，差异有统计学意义（P＜0.05）；治疗后两组HR、LVESV、BNP、hs-CRP均低于治疗前，且观察组低于对照组，差异有统计学意义（P＜0.05）；两组均未发生严重不良反应。结论 不同剂量阿托伐他汀治疗心梗后无症状心衰均具有一定的疗效，40 mg/d阿托伐他汀治疗效果优于 20 mg/d，更有助于改善患者心功能指标，未增加不良反应，值得临床应用。
Abstract:
Objective To analyze the effect of different doses of atorvastatin on asymptomatic heart failure after myocardial infarction.Methods 120 patients with asymptomatic heart failure after myocardial infarction diagnosed and treated in our hospital from January 2018 to January 2019 were selected and divided into a control group and an observation group by random number table method, 60 cases each, and the control group was given 20 mg/d Atorvastatin treatment. The observation group was given 40 mg/d atorvastatin treatment. The total effective rate, heart rate (HR), left ventricular ejection fraction (LVEF), and end ventricular systolic volume (LVESV) were compared between the two groups.Plasma BNP, high-sensitivity C-reactive protein (hs-CRP) levels.Results The total effective rate of treatment in the observation group was 93.33%, which was higher than 81.67% in the control group,the difference was statistically significant (P<0.05). After treatment, the LVEF of the two groups was higher than before the treatment, and the observation group was higher than the control group,the difference was statistically significant (P<0.05); HR, LVESV, BNP, and hs-CRP were lower in the two groups after treatment than before treatment, and the observation group was lower than the control group,the difference was statistically significant(P<0.05); No serious adverse reactions occurred in the groups.Conclusion Different doses of atorvastatin have a certain effect in the treatment of asymptomatic heart failure after myocardial infarction. The effect of atorvastatin at 40 mg/d is better than 20 mg/d, which is helpful to improve the cardiac function of patients without increasing. Adverse reactions are worthy of clinical application.
